What makes this form work
One question per screen suits a phone
An RSVP arrives by text or from a link in a bio. One question at a time is the format that gets finished on a small screen with one thumb.
Declining is a first-class answer
"Sadly I cannot make it" sits beside the yes, worded so nobody feels they have to explain. A form that only handles yes gets silence instead of a no.
Dietary needs are asked once, plainly
The kitchen needs the detail and the guest needs one box. "The kitchen would much rather know now" gets more back than "any dietary requirements".

What should an RSVP form ask?
Whether each person is attending, who is in their party by name, a meal choice if you are seating people, dietary needs, and a contact number for the day. A song request costs nothing and people enjoy it.
How do I stop people who decline seeing meal questions?
This form words the later questions so a declining guest can skip them in one tap. If you need questions to appear and disappear based on an earlier answer, that is conditional logic, which this builder does not have today.
Is a conversational form better than a normal one for RSVPs?
