What makes this form work

It asks for a suburb, not a home address

Commute distance is the only thing a street address tells you at this stage, and a suburb answers it without holding data you have no use for yet.

Work rights are asked the legal way

"Are you legally entitled to work here" and "will you require sponsorship" are both fair. "What is your citizenship status" and "where were you born" are not, and they are absent.

Expected salary, never salary history

What somebody wants is a useful filter. What they earned is prohibited in a growing number of places and anchors offers unfairly everywhere else.

Questions people ask

What questions are illegal on a job application?

Broadly: age beyond confirming an applicant is over 18, marital or family status, religion, citizenship or place of birth, disability before an offer, and in many places arrest records and salary history. Rules differ by jurisdiction, so check yours.

Can I ask about work authorisation?

Yes, if you ask it properly. "Are you legally entitled to work here" and "will you now or in the future require sponsorship" are both acceptable. Asking about citizenship status is not.

Should the resume be required?
