What makes this form work
Order lines are a table
One row per product, with item code and quantity in their own columns. A free-text order box is how the wrong thing gets picked.
Billing is asked, not assumed
The delivery address and the address the invoice goes to are different more often than not in trade. One question saves the credit team a phone call.
The purchase order number is on the form
If the buyer's accounts team needs a PO on the invoice, the moment to capture it is now. Chasing it after invoicing is what turns a 30 day term into 60.

What should a product order form include?
Customer and contact details, the products with quantities, a delivery address and method, and billing details. For trade, add an account number and a purchase order field.
Can this form take payment?
