Rental application form
One application per adult, covering everything an agent assesses. It is long because a short rental application just becomes three emails afterwards.

The form itself
A static copy of the finished form, exactly as it renders. Nothing here can be filled in or submitted, so read it as a reference and build your own from the button above.

What makes this form work
Rental history goes back three years
One previous address is what separates an application you can verify from one you cannot. Agents ask for it every time, so the form asks once instead.
Income is evidence, not a number
A payslip upload sits directly under the income question. Asking for the figure alone guarantees a follow-up email, and the applicant is already here.
Screening consent is explicit and signed
A named consent line plus a signature is what makes a database and credit check lawful. Burying it in the fine print is the most common mistake on a rental application.
Questions people ask
What information can a landlord ask for on a rental application?
Identity and contact details, rental history with previous agent contacts, employment and income, references, and consent to screening. Rules on what you may ask and store vary by state and country, so check your local tenancy law before you publish.
Do I need written consent to run a tenant check?
Yes. A background or credit check needs the applicant's written permission, which is why this form pairs a named consent checkbox with a signature.
Should each adult fill in their own application?
Yes. Income, rental history and consent are personal, and a joint application makes all three impossible to verify.

Everything a gym needs to sign somebody up in one sitting, including the health screening and the waiver signature that usually sends people back to paper.
Job application form
A job application that collects what actually decides a shortlist, and deliberately leaves out the questions that create legal risk.
