Sign a media interview consent form online
Signing a media interview consent form online should let a guest agree to each use separately, and know they can still pull out before the episode goes live. This one page form for a Hobart food and farming podcast has the guest type an X for audio, for video and for use of their full name, then sign with their name and the date, ten days before the recording.
The document, with its fields
The pages as a signer sees them, with the signature, name, title and date fields placed where each party has to complete them.
Interview Consent and Release
Kitehill Audio Pty Ltd (the Producer) would like to record an interview with you for episode 47 of Long Table Conversations, a weekly podcast about food, farming and the people behind them, hosted by Marnie Lockwood-Ferris. This form records what you agree to.
| Recording | Thursday 15 October 2026, 10.00 am to 11.30 am, at the Producer's studio, 4 Kelburn Mews, Hobart, or by video call if you prefer |
|---|---|
| Topic | Your work restoring heritage apple varieties, and running an orchard through a drought year |
| Episode | About 45 minutes after editing, planned for release on Monday 9 November 2026 |
Recording, editing and distribution. The Producer edits the recording for length and clarity without changing the meaning of what you say, and publishes the episode free on its website and podcast platforms, where it may stay available indefinitely, with clips of up to 60 seconds on its own social media. The Producer owns the copyright in the recording. No payment is made.
Your choices. Choose each item separately. For each one you agree to, type an X in its box. An empty box means no, and the Producer follows your choice without asking again.
| X | I agree that |
|---|---|
| Audio. My voice may be recorded and published in the episode and its audio clips. | |
| Video. The studio camera may film me, and the video may be published as a video version and video clips. | |
| Name used. My full name may be used in the episode, its title, its show notes and its promotion. Without this, I am introduced by first name only. |
Your right to withdraw. You may withdraw from the episode for any reason at any time before it is published, by emailing producer@kitehillaudio.example. The Producer then deletes the recording and does not publish it. You may also ask, before publication, for a part of the conversation to be removed. Your contact details are used only to manage the episode and are handled under the Australian Privacy Principles.
Consent. I have read this form. I agree to take part in the interview on the terms above and as chosen in the boxes. I am 18 or older.
Who signs what
- Esperanza Wilding, podcast guest
- Three optional checkboxes: audio, video, name used · Name, signature and stamped date · Email link
The signing journey
Step 1
Six fields on one page
Three consent boxes in a table, then a name, a signature and a date at the foot of the page.
Step 2
Three separate choices
Each box is optional. Esperanza typed an X in all three, so the signed PDF shows three X marks; any left empty would have meant no.
Step 3
Her name from the invitation
The name field fills from the recipient record, so the printed name is the one the producer invited.
Step 4
Signed before the recording
She signs on 5 October 2026, ten days before the recording on Thursday 15 October, and the certificate records the exact time.
Every field, and who fills it
Esperanza Wilding types an X in each of the three boxes in the consent table: audio, video and name used. Her name fills from her recipient record on the line below, she draws her signature, and her date stamps itself beside the printed episode line. The recording on Thursday 15 October 2026, the planned release on Monday 9 November 2026, the editing and distribution terms and the right to withdraw are printed by the producer.
How to adapt this envelope
An audio only podcast removes the video box. A show interviewing two guests together sends each their own envelope, because each guest chooses for themselves. A producer who wants a countersignature can add themselves as a second signer, though a consent form does not need one. A guest under 18 needs a parent or guardian added as a signer.
Why an electronic consent works here
Australian electronic transactions law accepts an electronic signature that identifies the person and shows their intention by a reliable method. The producer owns the copyright in the recording under the Copyright Act 1968, and handles the guest's contact details under the Australian Privacy Principles, both of which the form states.
What makes this signing flow work
Audio, video and name are three decisions
A guest happy to be heard may not want to be filmed, or named in a title that search engines index. Three optional boxes with their own wording let her say yes to each separately, and the signed copy records three X marks rather than one blanket consent.
The right to withdraw is printed above the signature
Guests agree more readily when they know they can change their mind. The form says plainly that she can withdraw any time before publication by email, and the producer will delete the recording, so her signature is given with that right in front of her.
One signer, no countersignature
The form is the guest's consent, not a contract both sides negotiate, so only the guest signs. The certificate's single signer row, with her email, IP address and time, is all the producer needs to show what was agreed and when.
Questions people ask
Does a podcast guest need to sign a consent form?
It is not always legally required, but a signed form avoids arguments later about what the guest agreed to. It records whether the guest's voice, image and name may be used, and on what terms, at the time the guest agreed, with the certificate showing when.
Can a guest sign the interview consent from anywhere?
Yes. The link arrives by email and opens in any browser, on a phone or a computer, with no account. On a narrow screen the three consent boxes move into a panel above the form, so they are easy to fill in before signing.
What if the guest does not sign before the recording day?
The link stays valid for 30 days from when the envelope was created, which the app does not let the producer change. If the recording is close, the producer can resend the invitation to the same address or host the guest's signing in person on the day.
What happens if the guest declines the consent form?
The envelope moves to declined and the producer is emailed with the guest's reason, if they gave one. Nothing is stamped and no certified copy exists, so the producer should not record or publish the interview on these terms, and can talk to the guest about what would work instead.
Can the guest withdraw after signing?
Yes, before publication. The form says the guest can email the producer at any time before the episode is released and the recording will be deleted and not published. After publication the producer considers requests to correct or remove content.
What does an empty box mean on this form?
It means no. The form says so in the line above the table, and the producer follows the choice without asking again. With the video box empty, for example, the studio camera would stay off for the session and only the audio version of the episode would be published.

Sign a model release form online
Signing a model release form online for paid work has to record more than consent to be photographed: the shoot date, each use the model agrees to, how long, and the fee. This two page release for a swimwear catalogue shoot in Byron Bay has the model confirm her name, type her agency or none, type an X for web use and for print use, and sign before the photographer countersigns.
Sign a location release online
Signing a location release online has to show that the person granting filming owns the property, which parts of it the crew may use, and how it will be put back. This two page release for three days of filming at a farmhouse near Ballarat has the owner type the areas she wants excluded, sign with her name and date, and the producer sign a four line company block, all before the first filming day.
Sign a sponsorship agreement online
Signing a sponsorship agreement online is about pinning down the benefits, because that table is what the sponsor will check the festival against months later. This five page major partner agreement for a Victorian twilight festival has the sponsor's marketing director initial the eight row benefits table behind an emailed code and sign a four line block, then the festival director signs.
