Sign a school camp consent form online
Signing a school camp consent form online has to do more than say yes: the teachers taking a child away overnight need the medications, allergies and diet from the parent, in the parent's words, before the due date. This three page form for a Victorian Year 6 camp has the parent type all three, choose swimming with a typed X, and sign with their name and the date.

School Camp Consent Form
This form asks a parent or carer to give consent for Rory Arkwright, Year 6, room 6B to attend the three day Year 5 and 6 camp, and to give the school the medical and dietary details the camp staff need. It is returned by Friday 30 October 2026. A student whose form has not been returned by that date cannot be placed in a cabin or an activity group and will not attend.

2. Program
| Day | Morning | Afternoon | Evening |
|---|---|---|---|
| Wednesday | Travel, cabin set up | Canoeing on the lake, orienteering | Campfire and night walk |
| Thursday | High ropes course | Lake swim, archery | Group performance night |
| Friday | Bush walk, 5 km | Clean up, travel home |
3. Activities and their risks
Every activity is run by qualified camp instructors with teachers present, using the camp's own equipment, which is checked before each session. The main risks are falls and sprains on the ropes course and the bush walk, cuts and scrapes, and the cold and fatigue of a busy outdoor program. Helmets and harnesses are worn on the ropes course and approved life jackets in every canoe.
The lake swim on Thursday afternoon takes place in a roped area no deeper than 1.5 metres, with two qualified lifeguards in the water and a teacher on the bank for every ten swimmers. Students are grouped by the swimming ability their parents report on page 2, and no student swims without the parent's permission in that section.
4. What to bring
A sleeping bag and pillow, warm clothes for the evenings, a raincoat, closed toe shoes that may get wet, a hat, sunscreen, a water bottle, toiletries, and a labelled bag of any medication with its instructions, handed to Mr Pettigrew at the coach. No phones, tablets, money or food from home.
5. Medical and dietary details
This section is typed by the parent in their own signing session. The school prints nothing here because only the parent can tell the camp staff what is true this week. Write "none" in any box that does not apply, rather than leaving it blank. These details are seen only by the teacher in charge, the camp first aid officer and the camp kitchen, and are destroyed when the camp is over.
6. Medication at camp
All medication travels in the labelled bag in its original packaging with the pharmacy label, and is kept and given by the teacher in charge at the times typed above, who records each dose. A student with an asthma action plan or an anaphylaxis management plan brings their reliever or adrenaline autoinjector, and a second device stays in the camp first aid room. The school already holds Rory's current medical record, and anything typed here is added to it for the duration of the camp only.
7. Swimming permission
If you give permission for Rory to take part in the lake swim described in section 3, type an X in the box below. An empty box means no swimming; Rory will join the archery group on the bank for that session instead and will not be asked why.
| X | Permission |
|---|---|
| Rory may swim in the roped area of the lake under the supervision in section 3. He can swim 50 metres without stopping and is comfortable in water out of his depth. |
8. Emergency contacts
The school uses the emergency contacts on Rory's current enrolment record. If they have changed since the start of the year, update them with the office by Friday 30 October 2026 so the camp list is right when it is printed on Monday 16 November 2026.
9. Behaviour on camp
The school's student engagement policy applies at camp exactly as it does at school. A student whose behaviour puts themselves or others at risk may be sent home at the parent's cost, after the teacher in charge has spoken to the parent by phone.
10. If Rory is unwell or injured
The camp first aid officer and the teachers give first aid. If Rory needs a doctor, a teacher takes him to the medical centre in Tarrawonga, eight minutes away, and calls you. In an emergency an ambulance is called first and you second; a teacher travels with him and stays until you arrive. Ambulance costs are the family's, so families without ambulance cover may wish to arrange it before camp.
11. Going home early
A student who is homesick is supported by their cabin teacher first, and goes home early only after a conversation with the parent. A student who is too unwell to continue is collected by a parent or a person the parent names on the phone, who shows identification at the camp office.
12. Photographs
Teachers take photos for the camp slideshow at the end of term assembly. The school's existing photo consent on Rory's enrolment record applies; this form does not change it.
13. Cancellation and refunds
If the camp is cancelled by the school, including for extreme weather or a fire danger rating that closes the camp, the full $395.00 is refunded. If Rory withdraws after Friday 30 October 2026, $160.00 is refunded, because the camp charges the school for every bed booked after that date.
14. Parent's consent
I am the parent or carer of Rory Arkwright. I have read the program and the activity risks above. I give consent for Rory to attend the Year 5 and 6 camp from Wednesday 18 to Friday 20 November 2026 and to take part in every activity listed, except the lake swim unless I have typed an X in section 7. The medical and dietary details I typed on page 2 are complete and correct, and I will call the school office if anything changes before the coach leaves.
In an emergency where I cannot be reached, I authorise the teacher in charge to consent to medical treatment that a doctor or paramedic advises is necessary for Rory's health.
Who signs what
- Delphine Arkwright, parent of Rory Arkwright
- Three text fields on page 2: medications, allergies, dietary needs · Optional checkbox on page 2: swimming permitted · Name, signature and stamped date on page 3 · Email link
The signing journey
Step 1
Seven fields, one parent
Three medical and dietary fields and a swimming box on page 2, then a name, a signature and a date on page 3.
Step 2
The school writes nothing medical
Section 5 is left blank on purpose. Each field carries a label telling the parent what to type, including the word none when nothing applies.
Step 3
Swimming is a choice
The swimming box is optional. Delphine typed an X, which prints as an X; leaving it empty would have kept Rory in the archery group on the bank.
Step 4
Signed before the due date
The form asks for a reply by Friday 30 October 2026, and the signature is stamped Oct 5, 2026, well before it.
Every field, and who fills it
Delphine Arkwright is the only recipient. On page 2 she types Rory's medications with the dose and time, his allergies, and his dietary needs, each on its own ruled line, and types an X beside the swimming permission. On page 3 her name prints from the recipient record, she draws her signature once, and the date stamps itself. The camp dates, the $395.00 cost, the program, the 1 to 8 supervision ratio and the $160.00 refund after the due date are printed by the school.
How to adapt this envelope
A school that wants both parents to consent adds the second parent with their own name, signature and date, and leaves the medical fields with the first. A camp with no water activity removes the swimming box. A school sending forms to a whole year level sends one envelope per student, because each envelope is one document and its own set of signers, and the status page then shows which families have not yet signed.
Why an electronic signature works here
The Electronic Transactions Act 1999 and the matching Victorian Act accept an electronic signature that identifies the person and shows their intention by a reliable method. Victorian government schools must also have informed consent from parents for excursions, which this form records together with the student information the policy asks schools to hold.
What makes this signing flow work
Medical details typed by the person who knows them
The camp's first aid officer reads Rory's medications, allergies and diet exactly as his parent typed them, on the same page she signed. A school cannot pre fill these fields, so stale information from the start of the year cannot slip into the signed copy.
Swimming is opt in
A lake swim is the riskiest part of the program, so it is the one activity that needs its own answer. An optional box with printed wording that empty means no gives the parent a real choice without blocking the rest of the consent.
A due date the certificate can prove
Cabins and activity groups are set from the forms returned by the due date. The certificate's signed time, printed in UTC, shows the consent arrived before Friday 30 October 2026, which settles any question about a late form.
Questions people ask
Is an online camp consent form as good as a paper one?
For the school's records, yes. An electronic signature is recognised under Australian electronic transactions law when it identifies the parent and shows their agreement. The signed PDF is certified so it cannot be altered afterwards, and the certificate records the parent's email, signing time and IP address.
Can a parent sign the camp form from work or overseas?
Yes. The link works in any browser, on a phone or a computer, and no account is needed. On a narrow screen the typed fields move into a panel above the document, so a parent can enter the medical details comfortably on a phone.
How long does a parent have to return the form?
The school's own deadline is the due date printed in the form, here Friday 30 October 2026. Separately, the signing link expires 30 days after the envelope was created, and that period cannot be changed in the app, so a school should send forms well inside it.
What if a parent types the wrong medication?
The signed copy cannot be edited, so the parent calls the school office, as clause 14 asks, and the teacher in charge updates the camp medical record. If the change is significant, the school can send a fresh one page update for the parent to sign.
What happens if a parent declines the consent form?
The envelope moves to declined and the school is emailed with the reason, if one was given. Nothing is stamped and the student is not placed in a cabin. The school can speak with the family and, if they change their mind, send a new form.
Who sees the medical details a parent types?
The form limits them to the teacher in charge, the camp first aid officer and the camp kitchen, and says they are destroyed when the camp is over. When the last signature lands, the certified PDF is emailed to the parent and to the school staff member who sent it, and to nobody else.

Make one like thisOther signing examples
Sign a childcare enrolment agreement online
To sign a childcare enrolment agreement online, the parent has to supply the facts only a parent can give, the people allowed to collect the child and the consents for photos and sunscreen, before the centre accepts. This five page agreement for a three year old at a South Australian long day care centre has the parent type three pickup names, choose two consents as typed X marks and sign, and the director signs after.
Sign a tutoring agreement online
Signing a tutoring agreement online works best when each side supplies its own details and neither waits for the other: the parent knows the student's year level, the tutor knows their ABN. This two page agreement for ten Tuesday English sessions in New South Wales puts one typed field and one signature in front of each, with both invited at the same moment.
Sign a student code of conduct online
Signing a student code of conduct online is an acknowledgement, not a contract: the school needs to show that the student read the rules and that a parent read them too, each in their own name. This six page Queensland code ends with two columns on the last page, the Year 10 student signing first from a school email and the parent second from her own.
