Education and childcare e-signature examples, with the audit trail

School and childcare paperwork usually needs more than one adult to sign for one child, and the school needs to know both did. Each example here is a real envelope signed end to end, with its fields, its signing order and what the certificate records.

Who signs for a child, and in what order

School and childcare envelopes often need a parent as well as the person the document is about. A Year 7 enrolment contract is signed by both parents and then the school, each parent checked with an emailed one time code. A childcare enrolment, an after school care agreement and a tutoring agreement each take a parent and the provider; the tutoring envelope switches the order off so neither waits for the other. A camp consent needs the parent alone. A student code of conduct takes the student and then a parent, and a device loan the parent, the student and the school's IT coordinator. A work placement and an apprenticeship contract each take four signers. Older students sign for themselves: a scholarship acceptance, and a student accommodation agreement with a guarantor.

Fields that carry the decisions

An enrolment contract asks parents to agree to things that matter later: the fee schedule, the notice needed to withdraw, photo and media consent and the medical authority. The example places each parent's initials side by side under the fee table, so both mark the fees as read rather than relying on a final signature to cover them, then a signature and a stamped date for each parent on the last page. The school signs a block with its registrar's name, title, school and the date, which is stamped at signing rather than typed. Every field and who fills it is listed on the page, so the envelope can be rebuilt from it.

What signing settles

Signing records that each person agreed and when, with the verification they were given and a numbered event log on the certificate of completion. It does not change what the law requires of a school or childcare service, and a contract term that is unfair is not made fair by being signed. All eleven examples are set in Australia, and six are Australian pages that name the rules that apply. Every school, family and business is invented. Start from the document closest to yours and change the signers and the verification to match.
