Health and care consent e-signature examples, with the audit trail

Consent has to be informed, specific and recorded, and the record has to show who agreed, to what and when. Each example here is a real envelope signed end to end, with its fields, its order and what the certificate of completion holds.

Who signs, and in what order

Most of these envelopes take two signatures in order: the patient or client first, then the clinician who explained the treatment. The surgical, dental, cosmetic, physiotherapy and vaccination consents work that way, and on the physiotherapy consent the physiotherapist countersigned twenty minutes after the patient. Three envelopes take three signers. The NDIS service agreement adds a plan nominee, and the home care agreement is sent to all three parties at once with the order switched off, because nobody needs to wait for anyone else. The telehealth and patient privacy consents need one signer.

Choices inside the document

A consent form often asks the signer to opt in or out of particular things, such as blood products, dry needling, sharing records with another provider or being contacted for future studies. These are checkbox fields, and the signer types a mark into each box, which prints as an X in the signed PDF. A box set as required must be filled before the signer can finish, so where an empty box should mean no, the box has to be optional. Several pages explain how they handled that. Optional statements on the research and privacy consents are marked as optional.

What the certificate records

Every envelope comes back certified, with a certificate showing each signer, the verification they were given, the time they signed, the consent they accepted and a numbered event log. The surgical, cosmetic, aged care and counselling envelopes add an emailed one time code for the first signer; the rest rely on the email link. A signed form records agreement; it does not replace the conversation about risks, which the documents say happened first. Six of the twelve are Australian pages that name the law and guidance that apply. Every clinic and person is invented.
