Sign a home care agreement online
A home care agreement has to fix the weekly schedule, the hourly prices and how carers get into the house, and it has to bind everyone the schedule depends on. This four page private agreement for an older woman in Adelaide is signed by her, by her son who covers two days himself, and by the provider, all invited at once.
The document, with its fields
The pages as a signer sees them, with the signature, name, title and date fields placed where each party has to complete them.
Home Care Agreement
1. Who this agreement is between
| Client | Doreen Halloway, of 23 Fisher Street, Fullarton SA 5063 |
|---|---|
| Family carer | Graham Halloway, Doreen's son, of 9 Marlborough Street, Henley Beach SA 5022, who shares Doreen's care with Kindred and is the first contact for the care team |
| Provider | Kindred Home Support Pty Ltd, ABN 26 640 381 117, of 150 Unley Road, Unley SA 5061 |
| Start date | Monday 12 October 2026 |
| Funding | Paid privately by Doreen. No government aged care funding is used for these services |
2. Why the three of us sign
Doreen receives the care and pays for it, so the agreement is hers. Graham signs too because the weekly schedule depends on him: he does the Saturday visit and the Wednesday shopping himself, and Kindred's visits are built around his. Kindred's care coordinator signs for the Provider. All three are invited at the same time and may sign in any order.
3. The weekly visit schedule
| Day | Time | Who | What happens |
|---|---|---|---|
| Monday | 8:00 am to 9:00 am | Kindred | Shower, dressing, breakfast, morning medicines from the Webster pack |
| Tuesday | 8:00 am to 8:45 am | Kindred | Morning routine, light tidy, bed made |
| Wednesday | 8:00 am to 9:00 am | Kindred | Shower, dressing, breakfast, medicines |
| Wednesday | 10:00 am to 12:00 pm | Graham | Shopping at Burnside Village with Doreen, pharmacy pick up |
| Thursday | 8:00 am to 8:45 am, 2:00 pm to 4:00 pm | Kindred | Morning routine; afternoon cleaning, washing and changing the bed |
| Friday | 8:00 am to 9:00 am | Kindred | Shower, dressing, breakfast, medicines, and a walk to the corner if the weather allows |
| Saturday | 9:00 am to 1:00 pm | Graham | Breakfast together, garden, cooking two meals for the freezer |
| Sunday | 8:30 am to 9:15 am | Kindred | Morning routine, then Doreen's neighbour drives her to church |
Kindred's weekly total is 7 hours and 15 minutes. The schedule is reviewed with Doreen and Graham after the first four weeks and then every three months.
4. The tasks Kindred's carers do
| Area | Tasks |
|---|---|
| Personal care | Help in and out of the shower using the shower chair and rails, washing, dressing, hair, applying moisturiser to her legs |
| Medicines | Prompting Doreen to take the tablets in the pharmacy packed Webster pack, and recording that she did. Carers do not choose, change or give medicines that are not in the pack |
| Meals | Preparing breakfast, and heating a frozen meal for lunch on Thursday |
| Household | Vacuuming, bathroom and kitchen cleaning, washing and hanging out clothes, changing the bed on Thursday |
| Wellbeing | Conversation, the Friday walk, noting anything unusual and telling Graham the same day |
Carers do not do nursing tasks such as wound care or injections, heavy gardening, ladder work or driving Doreen in their own car. If Doreen needs any of these, Kindred will say so and help arrange it.
5. Fees
| Item | Rate |
|---|---|
| Weekday visits, per hour | $72.00 |
| Saturday and Sunday visits, per hour | $94.00 |
| Public holiday visits, per hour | $128.00 |
| Usual weekly total, 6.5 weekday hours and 0.75 Sunday hours | $538.50 |
Visits are charged in 15 minute units for the time spent with Doreen. There is no joining fee, no travel charge and no exit fee. Kindred invoices Doreen fortnightly in arrears by email to Graham, with payment due within 14 days by bank transfer.
6. Cancelling a visit
Doreen or Graham may cancel a visit free of charge with 24 hours' notice by phoning 08 8271 4400. A visit cancelled with less notice is charged at half the usual rate, unless Doreen is in hospital. If Kindred cannot staff a visit it tells Graham as early as possible and offers another time.
7. Carers and continuity
Kindred rosters a small team of three carers to Doreen so she sees familiar faces, and tells her in advance if someone new will come. Every carer holds a current police check, a working with vulnerable people clearance, a first aid certificate and training in manual handling. Carers are Kindred's employees, not contractors.
8. Getting in: the key safe
Doreen keeps a spare front door key in a key safe so carers can let themselves in if she is still in bed or cannot get to the door. Kindred holds the code securely and gives it only to the carers rostered to her. Doreen types the location of the key safe and any instructions for using it here, in her own signing session. Kindred does not write the code on this agreement and Doreen should not type it here.
9. If Doreen does not answer
If a carer arrives and Doreen does not answer the door or the phone, the carer lets themselves in with the key safe. If she cannot be found or is hurt, the carer calls 000 first and then Graham. If the house is locked from inside, the carer calls Graham and then the police.
10. Privacy and records
Carers write a short note after each visit in Kindred's app, which Graham can read. Kindred keeps Doreen's personal information confidential, uses it only to deliver her care, and handles it under the Privacy Act 1988.
11. Complaints
Doreen or Graham can raise any concern with the care coordinator, Marguerite Ashby, on 08 8271 4400 or at care@kindredhome.example. Kindred replies within two business days. If they are not satisfied, they may contact the Health and Community Services Complaints Commissioner in South Australia, or Consumer and Business Services for a problem with the contract or the fees.
12. Health information Kindred holds
| Item | Detail |
|---|---|
| GP | Dr Hamish Royce, Fullarton Medical Clinic, 08 8379 2210 |
| Pharmacy | Glen Osmond Road Pharmacy, which packs the Webster pack weekly |
| Allergies | Sulphur based antibiotics |
| Mobility | Walks with a four wheeled frame; uses the shower chair for every shower |
13. Changes and ending the agreement
Doreen may change or reduce the schedule with a week's notice, or end the agreement at any time with 14 days' notice in writing, with no exit fee. Kindred may end it with 28 days' notice in writing, or straight away if a carer is seriously threatened or harmed. Kindred may change its rates once a year with 30 days' written notice.
14. Insurance and responsibility
Kindred holds public liability insurance of $20,000,000 and workers' compensation insurance for its carers. Doreen keeps her home safe for carers to work in, including keeping the path clear and her dog in the laundry during personal care. Nothing in this agreement limits Doreen's rights under the Australian Consumer Law.
15. Agreement
Doreen, Graham and Kindred agree to this home care agreement. All three are invited together and may sign in any order. Each receives the certified copy once all three have signed.

The signing journey
Step 1
Six fields across two pages
A text field for Doreen on page 3, then signatures and dates for Doreen and Graham and a block for the coordinator on page 4.
Step 2
All three invited together
Sign in order is off. The certificate's event log shows three invitations emailed within two seconds of each other, then Sent for signing.
Step 3
Doreen types where the key is
She types Left of front door, behind the lavender pot. Slide cover down, and nothing else. The page asks her not to type the code.
Step 4
Anyone can finish last
In this run Doreen, Graham and the coordinator signed in list order, but the envelope would have completed whichever of them signed third.
Every field, and who fills it
Doreen Halloway types the key safe location on the ruled line in clause 8 on page 3. The long note shrinks to fit its line rather than running past it. On page 4 she and Graham sign side by side with stamped dates, and Marguerite Ashby's block for Kindred Home Support sits below them with Name, Title, Company and Date. The schedule, the $72 and $94 hourly rates, the $538.50 usual weekly total and the cancellation rule are printed.
How to adapt this envelope
Where two adult children share the family visits, each becomes a recipient with their own signature, and the schedule table names them on their days. A client whose son holds a power of attorney can have him sign for her, with a text field stating the authority. A provider that wants its coordinator to sign only after the family adds the order switch and puts her last.
Why the certificate matters here
In home care, disputes tend to be about what was agreed: which tasks, which days, what a cancellation costs. The certified PDF fixes the schedule and prices, and the certificate shows that each of the three received their own link at the same time and signed from their own email address, with times and IP addresses for each.
What makes this signing flow work
The family carer signs because the schedule needs him
Graham does the Wednesday shopping and the Saturday visit himself, and the provider's 7 hours and 15 minutes a week are built around him. Making him a signer turns his part of the schedule into an agreed arrangement rather than a hope.
No order, because nobody countersigns anybody
The client, her son and the provider each agree to the same schedule independently. With the switch off, three links go out at once and nobody waits, which suits a family in different suburbs and a coordinator on the road.
The key safe note, but never the code
Carers need to know where the key safe is, and that detail should come from the person who lives there. A text field she fills puts it on page 3, while the printed wording tells her to leave the code off a document that is emailed to three people.
Questions people ask
Does a family carer have to sign a home care agreement?
Not usually, but here the schedule depends on him: two of the eight weekly slots are his. Signing makes those slots part of what everyone agreed. If a family member only wants copies, they can be sent the certified PDF by the client instead of signing.
Can all three sign on different days?
Yes. With Sign in order off, each link works independently until the envelope expires 30 days after it was created. The envelope completes when the third signature lands, whoever that is, and all three then receive the certified copy by email with the certificate as a separate download.
Why not put the key safe code in the agreement?
The signed PDF is emailed to three people and stored by each of them. A code in it would be in three inboxes for as long as the email exists. The provider keeps the code separately and gives it only to rostered carers, as clause 8 says.
Can the client sign on a tablet at home?
Yes. The portal works in any browser and needs no account. On a narrow screen the key safe line moves into a panel above the document. She types the note, draws her signature with a finger and accepts the electronic signing consent.
Is a home care agreement valid when signed electronically?
In Australia, yes, where the method identifies each signer and shows intention, which the Electronic Transactions Act 1999 accepts. The certificate records how each of the three was verified, here by email link, when they signed and from which IP address.
What happens if one of the three declines?
The envelope moves to declined and the provider is emailed with any reason typed. Any signatures already given stay in the working copy, but the agreement does not complete. The provider can adjust the schedule, void the envelope and send a new one.

Make one like thisOther signing examples
Sign an NDIS service agreement online
An NDIS service agreement belongs to the participant, so the signed copy should show that he agreed to it himself, that his plan nominee agreed too, and that the provider signed last. This six page agreement for daily living, travel training, basketball and cooking supports puts all three signatures on page 6, with the participant signing first.
Sign a physiotherapy consent form online
A physiotherapy consent form has to capture the patient's answers to the safety questions before any needle goes in, and show the physiotherapist read those answers before treating. In this two page consent for low back pain with dry needling, the patient marks one of five screening questions with an X and signs, and the physio countersigns twenty minutes later.
Sign a dental treatment consent online
A dental treatment consent has to show that the patient understood the risks, had questions answered and knew what the plan would cost before any work started. This three page consent for a root canal and crown on one molar carries the full fee table with item numbers, three statements the patient marks with an X, and the dentist's countersignature.
