Sign an NDIS service agreement online
An NDIS service agreement belongs to the participant, so the signed copy should show that he agreed to it himself, that his plan nominee agreed too, and that the provider signed last. This six page agreement for daily living, travel training, basketball and cooking supports puts all three signatures on page 6, with the participant signing first.
The document, with its fields
The pages as a signer sees them, with the signature, name, title and date fields placed where each party has to complete them.
NDIS Service Agreement
1. Who this agreement is between
| Participant | Darcy Pemberton, NDIS number 431 882 106, of 15 Gilbert Street, Kings Meadows TAS 7249 |
|---|---|
| Plan nominee | Celia Pemberton, Darcy's mother, appointed by the NDIA as his plan nominee, of the same address |
| Provider | Tidewater Community Supports Pty Ltd, ABN 39 617 054 220, a registered NDIS provider, of 88 Brisbane Street, Launceston TAS 7250 |
| Plan dates | 1 July 2026 to 30 June 2027 |
| Plan management | Plan managed. Invoices go to Bassgrove Plan Management, who pay the Provider from Darcy's plan |
| Agreement dates | 12 October 2026 to 30 June 2027, unless ended earlier under clause 14 |
2. About this agreement
This agreement is made for the purpose of providing supports under Darcy's NDIS plan. It sets out which supports the Provider will deliver, how much they cost, how they are paid for, and what Darcy, his nominee and the Provider each agree to do. It is written to be easy to read. An easy read summary with pictures was given to Darcy at the planning meeting on 28 September 2026, and his support coordinator went through it with him.
This agreement is made in line with the NDIS Act 2013, the NDIS Practice Standards and the NDIS Code of Conduct. It does not change Darcy's rights under the Australian Consumer Law or any other law.
3. Darcy's goals
| 1 | To get ready for work each morning on his own, with less prompting over the year |
|---|---|
| 2 | To travel to his supported job at the Launceston nursery by bus without a support worker by June 2027 |
| 3 | To join the Tuesday evening community basketball team and make friends there |
| 4 | To learn to cook three simple meals for himself |
4. How Darcy likes supports delivered
Darcy prefers the same two support workers, Liam and Hannah, who he met at the planning meeting. He likes clear plans written on the whiteboard in the kitchen the night before, no surprises in the morning, and to be spoken to directly rather than through his mum. He does not like loud music in the car.
5. The supports
Rates are the Provider's agreed prices. They are at or below the price limits in the NDIS Pricing Arrangements in force on the day each support is delivered, and include no GST, because NDIS supports to a participant are GST free.
| Support | When | Hours a week | Rate |
|---|---|---|---|
| Assistance with daily life, self care | Monday to Friday, 6:45 am to 8:15 am | 7.5 | $68.50 an hour |
| Community participation, travel training | Monday and Wednesday, 8:15 am to 9:30 am | 2.5 | $68.50 an hour |
| Community participation, basketball | Tuesday, 6:00 pm to 8:30 pm | 2.5 | $73.20 an hour |
| Capacity building, cooking skills | Thursday, 4:00 pm to 6:00 pm | 2.0 | $68.50 an hour |
| Provider travel | Up to 20 minutes each way to the first support of a day | as used | at the support rate |
6. Estimated cost for the agreement period
| Item | Weeks | Estimate |
|---|---|---|
| Daily life and travel training, 10 hours at $68.50 | 37 | $25,345.00 |
| Basketball, 2.5 hours at $73.20 | 37 | $6,771.00 |
| Cooking skills, 2 hours at $68.50 | 37 | $5,069.00 |
| Estimated total, before provider travel | $37,185.00 |
This estimate assumes every support is delivered every week. The Provider checks it against the funding in Darcy's plan every month and tells Darcy and his nominee in writing if spending is running ahead, so supports never stop without warning because funding has run out.
7. Payment
The Provider invoices Bassgrove Plan Management fortnightly, showing every support delivered with its date, start and finish time and rate. A copy goes to Celia. Darcy will never be asked to pay anything out of his own money for a support in this agreement.
8. Cancellations and changes
| What happens | Charge |
|---|---|
| Darcy or Celia cancels with 2 clear business days' notice or more | No charge |
| Short notice cancellation or no show, as defined in the NDIS Pricing Arrangements in force at the time | Up to 100 per cent of the agreed price, only where the Pricing Arrangements allow it and the worker could not be given other work |
| Darcy is unwell or in hospital | No charge if the Provider is told before the worker sets out |
| The Provider cancels | No charge, and another time is offered in the same week |
The Provider keeps a record of every cancellation and shows it on the invoice, so Bassgrove Plan Management and Celia can check any cancellation charge.
9. Changes to this agreement
Any change to the supports, times or rates is agreed in writing between Darcy, his nominee and the Provider. The Provider gives at least 14 days' notice of any change in rates, which can happen only if the NDIS Pricing Arrangements change. If Darcy's plan is reassessed, this agreement is reviewed within 14 days of the new plan.
10. Darcy's and Celia's responsibilities
- Tell the Provider about anything that may affect a support, including changes to Darcy's plan, health or address.
- Give 2 clear business days' notice of cancellations where possible.
- Treat the Provider's workers with courtesy and respect, and keep the home safe for them to work in.
- Tell the Provider if this agreement or the plan manager changes.
11. The Provider's responsibilities
- Deliver the supports safely, at the agreed times, by workers who hold an NDIS worker screening clearance.
- Talk to Darcy directly, and in a way he understands, about every decision that affects him.
- Meet the NDIS Practice Standards and the NDIS Code of Conduct.
- Keep Darcy's information private and give Darcy and Celia copies of his records when they ask.
- Give fortnightly invoices that match the supports actually delivered.
12. Darcy's rights
Darcy has the right to choose who supports him and how, to be treated with dignity and respect, to be free from abuse, neglect and exploitation, to have his privacy respected, to have someone with him when he wants, and to complain without being treated differently. The Provider does not use any restrictive practice in delivering these supports.
13. Feedback and complaints
| Who to tell | How |
|---|---|
| The Provider | Talk to the service coordinator, Rahul Bhattacharya, on 03 6331 7720, or email feedback@tidewatercs.example. The Provider acknowledges a complaint within 2 business days and aims to resolve it within 21 days |
| NDIS Quality and Safeguards Commission | Free on 1800 035 544, or online. A complaint can go straight to the Commission at any time |
| An advocate | Darcy may ask an independent disability advocate to help him make a complaint |
14. Ending this agreement
Darcy, or Celia on his behalf, may end this agreement at any time by giving 14 days' notice in writing. The Provider may end it by giving 28 days' notice in writing, and will help Darcy find another provider before supports stop. If either party seriously breaches this agreement, the other may end it straight away. This agreement ends automatically on 30 June 2027, or earlier if Darcy's plan ends, unless it is renewed.
15. Privacy
The Provider collects Darcy's information to deliver his supports, shares it only with his plan manager, his support coordinator and others he or Celia agree to, and handles it under the Privacy Act 1988 and its privacy policy.
16. Contact details
| Who | Contact |
|---|---|
| Darcy | 0412 556 093 |
| Celia, plan nominee | 0419 227 380, celia.pemberton@tasmail.example |
| Provider, after hours | 03 6331 7700, answered 24 hours |
17. Darcy's weekly timetable
This is the timetable the supports in clause 5 make, written the way it goes on Darcy's kitchen whiteboard. It changes only when Darcy agrees, and the Provider gives him the new version a week before it starts.
| Day | Morning | Afternoon and evening |
|---|---|---|
| Monday | 6:45 shower, dress, breakfast with Liam. 8:15 bus practice to the nursery with Liam | Work at the nursery until 3:00 pm |
| Tuesday | 6:45 morning routine with Hannah | Work until 3:00 pm. 6:00 pm basketball at the Elphin Sports Centre with Hannah |
| Wednesday | 6:45 morning routine with Liam. 8:15 bus practice, Liam rides two stops behind | Work until 3:00 pm |
| Thursday | 6:45 morning routine with Hannah | Work until 3:00 pm. 4:00 pm cooking with Hannah: this term spaghetti, stir fry and omelette |
| Friday | 6:45 morning routine with Liam | Work until 3:00 pm |
| Weekend | No supports | Family time |
18. Incidents and emergencies
If Darcy is hurt, unwell or in danger during a support, the worker calls 000 first if needed, then Celia, then the service coordinator. Every incident is written up the same day and Celia is told about it within 24 hours. Incidents the law requires to be reported to the NDIS Quality and Safeguards Commission are reported within the required time, and Darcy and Celia are told what was reported.
19. Reviewing progress
The service coordinator meets Darcy and Celia every three months to look at progress towards each goal in clause 3, using the travel training log, the basketball attendance record and the cooking checklist. A short written summary goes to Darcy's support coordinator before his plan review in May 2027.
20. If Darcy's plan funding changes
If the NDIA changes Darcy's plan during the agreement period, the Provider meets Darcy and Celia within 14 days to match the supports to the new funding. Supports continue at the current level in the meantime unless the new plan has no funding for them, in which case the Provider explains which supports stop and from when.
21. Agreement
Darcy, Celia and the Provider agree to the terms of this service agreement. Darcy signs first, because it is his agreement and his supports. Celia signs second as his plan nominee and types her relationship to him. The Provider signs last. Each of them receives the certified copy once all three have signed.

The signing journey
Step 1
Six fields on the last page
A signature and date for Darcy, a signature, date and relationship line for Celia, and one block for the provider underneath.
Step 2
Darcy first
Sign in order is on. The participant is invited at send, and nobody else can sign until he has.
Step 3
Celia next, with her role in her words
Her link arrives when his signature lands. She types Mother, NDIA appointed plan nominee on the line under her date.
Step 4
The provider last
Rahul's invitation follows Celia's signature. He types Service Coordinator and the company name; his name and date fill themselves.
Every field, and who fills it
Darcy Pemberton signs in the left column on page 6 and his date is stamped. Celia Pemberton signs in the right column, her date is stamped, and she types her relationship on the line beneath. Rahul Bhattacharya's block for Tidewater Community Supports sits below both with Name, Title, Company and Date. Nothing else is typed: the supports from 7.5 hours of daily living to 2 hours of cooking, the rates of $68.50 and $73.20 an hour and the timetable are printed.
How to adapt this envelope
A participant without a nominee is a two recipient envelope, participant then provider. A participant who wants the agreement read to them can still sign through their own link; the easy read summary is given separately. For a self managed participant, the payment clause changes to invoices sent to the participant, and nothing about the signing order needs to change.
What makes this valid in Australia
The Electronic Transactions Act 1999 recognises an electronic signature that identifies the signer and shows intention, by a reliable method. Service agreements are made for supports under the National Disability Insurance Scheme Act 2013, and registered providers are regulated by the NDIS Quality and Safeguards Commission, whose Practice Standards and Code of Conduct the agreement commits the provider to.
What makes this signing flow work
The participant signs first
Clause 21 says Darcy signs first because it is his agreement and his supports. The order switch makes that true in the record: the certificate's event log shows his signature before his mother's invitation was even sent.
The nominee says who she is
A plan nominee signs in a defined role. The text field under her date lets her state it, Mother, NDIA appointed plan nominee, so the signed page shows the capacity she signed in rather than leaving a reader to guess.
Rates, cancellations and goals on the same document
The supports table, the $37,185 estimate for 37 weeks, the short notice cancellation terms and Darcy's four goals are all on pages the three of them signed, so a plan manager checking an invoice can point to the agreed rate.
Questions people ask
Does an NDIS service agreement have to be signed?
A written agreement is strongly encouraged so everyone knows which supports are delivered, at what price and how changes and cancellations work. A signed copy also lets a plan manager check invoices against the agreed rates. This one is signed by the participant, his nominee and the provider.
Why does the participant sign before the nominee?
Because the agreement is his. Putting him at order 1 means his own agreement is recorded first, and the nominee's signature follows as support for his decision. The event log shows the order, and the certificate lists all three signers with their times.
Can a participant with an intellectual disability sign electronically?
If he understands what he is agreeing to, yes, and the easy read summary and the support coordinator's walkthrough help with that. Where a participant cannot make the decision, the person with legal authority signs instead. The signing tool records who signed, not whether they had capacity.
What does the nominee's relationship line add?
It records the capacity she signs in, typed by her, directly under her signature. A provider cannot pre fill it, so the words Mother, NDIA appointed plan nominee are her own statement, which matters if her appointment is ever questioned.
Can the provider sign first and send it to the family?
It can, by putting the provider at order 1, but this agreement deliberately does not. Signing last lets the provider countersign an agreement the participant has already accepted, and avoids sending a family a document that looks as though it has already been decided for them.
What happens if the nominee declines?
The envelope moves to declined and the provider is emailed with any reason typed. Darcy's signature stays in the working copy but the agreement does not complete. The provider can meet the family, then void the envelope and send a revised agreement.

Make one like thisOther signing examples
Sign a home care agreement online
A home care agreement has to fix the weekly schedule, the hourly prices and how carers get into the house, and it has to bind everyone the schedule depends on. This four page private agreement for an older woman in Adelaide is signed by her, by her son who covers two days himself, and by the provider, all invited at once.
Sign an aged care service agreement online
An aged care service agreement signed by a family member has to show on what authority she signed, that she saw every page of services, prices and rights, and that the provider countersigned after checking that authority. This eight page Support at Home agreement does all three: the daughter types her authority, initials pages 2 to 7 behind an emailed code, and the care manager signs last.
Sign a research participant consent form online
A research consent form has to show that the participant read the information sheet, agreed to each statement separately, and was told one of them was optional. This four page information sheet and consent form for a six week sleep study puts six statements on the last page, each with its own box, followed by the participant's and the researcher's signatures.
