Camp registration form

Camps do not run one registration form, they run a packet: the booking, the health record, the medication authority and the collection list. This is all four on one screen, ordered the way the camp office actually needs them on the first morning. It is a classic form, because four documents in one only works if a parent can scroll back to what they already wrote.

We cannot give your child any medication, including paracetamol, antihistamines and an inhaler, until the table below is filled in and the medicine arrives in its original labelled box. That is the rule that keeps the first aid room legal, and there is no way around it on the morning.

What makes this form work

The collection list tells you to include yourself

The most common gap in a pickup list is the parent who filled it in, because it reads like a list of other people. One sentence fixes it, along with the warning that photo identification is checked every time, which stops the argument happening at four o'clock on a Friday.

The health question starts with a way to say no

The first option is that nothing applies. Most families tick it and move on in a second, and the ones who do not are answering a question rather than proving something. The follow up box then asks for triggers and what helps, which is what a camp leader can actually act on.

The minor injury question saves twenty phone calls

Some parents want to know about every scraped knee and some would rather hear at pick up. Asking once, in the health section, means the camp office is not guessing at four in the afternoon and no parent is either annoyed or alarmed by the call they get.

Questions people ask

Why are emergency contacts and the collection list two separate lists?

They answer different questions. Emergency contacts are who to ring when something happens, in order. The collection list is who is legally allowed to take a child off site, and staff will refuse anybody not on it. Merging them is how a grandparent ends up on a pickup list they were never meant to be on.

What does a camp need before it can give medication?

A written authority naming the medicine, the dose, the time and the reason, and the medicine itself in its original labelled packaging. That includes paracetamol and inhalers. Putting the table on the registration form means the authority arrives with the booking rather than in a text message at 8.25 on Monday.

How should a camp ask about swimming?

With three described bands rather than beginner, intermediate and advanced, which every family reads differently. Naming the distance and the confidence, and asking parents to answer for a bad day rather than a good one, is what puts a child in the right section of the roped pool.

Is one form per child or one per family better?

One per child. Siblings have different medications, different swimming ability and often different collection arrangements, and a shared form makes the first aid folder unreadable. Saying "one form per child" at the top saves the office splitting them by hand later.

Why does the form ask for the age on a specific date?

Because camp groups are set by age on the day, not by age at booking. This form asks for the date of birth and the age on 12 January. A child who turns nine the week before camp moves groups, and asking both means the office never has to work it out from a birthday in a spreadsheet.

What is the point of asking when a parent wants to be told about a scrape?

It sets the threshold before anything happens. This form asks, for a scrape or a bumped knee, when you want to hear from us. Some parents want a call every time and some want to hear at pick up. Agreeing it in advance stops both the unnecessary phone call and the complaint about not being told.

Swim school registration form with the level ladder on it

A swim school takes a whole term in one go: the class time, the level the child swims at now, and the health facts a teacher needs while standing at the edge of the water. This form collects all of it in fourteen fields and finishes with a guardian signing the emergency treatment authority, so no term starts on a verbal yes.

An outside school hours care service cannot start a child until it knows who may collect them and whether the subsidy can be paid. This registration settles both, with a four row collection table that nobody can add to at the gate and a subsidy section that asks for the two reference numbers the enrolment arrangement needs.
