What a swim school form has to settle before week one
Learn to swim runs on lanes, and a lane is a fixed number of places at a fixed half hour. So the form settles the timetable first, with a term, a tick list of days and a start time, because a family who can only do Saturday at nine is a different problem from one who can take any weekday. It then settles the lane itself, and since level names mean nothing between centres, the six options describe what the child can do. The third question is medical, and it is required, because a teacher reads it wet at the pool edge before the lesson starts.

Why is the health question a long text box rather than a tick list?
Because the useful part is what a teacher should do, not which box was ticked. Grommets, asthma and a seizure last winter each need a sentence of context.
