The two things an OSHC registration must settle
Everything else on this form is administration. The first thing that matters is who may take the child out the gate, and the second is whether the subsidy will be paid. The collection list is a four row table rather than a free text box because at five past three an educator is reading it at arm's length with twenty children behind them, and a paragraph is not readable in that moment. The custody question is separate and required, because the useful information is usually about a person who must not collect, and that never appears in a list of people who may.
The Australian subsidy pieces
Child Care Subsidy is paid against an enrolment arrangement, and an enrolment arrangement needs a customer reference number for the child and one for the claiming parent, which is why both sit in their own section with a paragraph above them saying where to find them. A family moving from another service already has a CCS enrolment ID, so that is asked as an optional third field rather than as a puzzle for the office to solve. The thank-you screen carries the part families forget: the arrangement has to be confirmed in the Centrelink online account or the full fee applies.
How to adapt this form
A long day care centre keeps the subsidy section word for word and replaces the session tick list with a booked days grid, since the day is not split into before and after school. A vacation care only service drops the class and teacher fields and turns the sessions question into a list of dated program days. An independent school running its own after school program removes the subsidy block entirely, because fees go on the school account, and adds a boarding house or bus route field where the collection question sits.

The child CRN and the claiming parent CRN sit side by side under a note pointing at any Centrelink letter. Services that ask for one number, or bury it among health questions, spend the first fortnight chasing the other by phone.
The custody question is required and asks for the negative
It names court orders, parenting plans and anyone who must not collect, and it asks for the word none rather than allowing a blank. A blank field and a deliberate none look identical in an export, and only one of them is information.
The action plan is a file upload, not a tick box
A signed plan is the document educators follow, so the form takes it as a file with a description that says PDF or a clear photo. The medical text box above it carries the context, but the plan itself is the thing that hangs in the room.

How many fields does an after school care registration need?
Twenty here, and sixteen of them are required, which is heavy for a form but light for a service that is legally responsible for a child until six fifteen. The welcome screen warns that the subsidy section needs a Centrelink letter nearby, because the abandoned registrations are almost always the ones started without it.
Why is the collection list a table with four fixed rows?
Because a table renders exactly the rows it is given and cannot grow at the gate. Four covers two parents and two others, which matches how most families actually arrange pickup. A service that needs more raises the row count in the builder, but the number is a deliberate decision rather than an open invitation.
What about consent when the child is a minor?
A parent or guardian gives it, and this form separates the three things they are agreeing to rather than bundling them: two weeks notice to cancel, absence charging, and emergency medical attention. All three are required, because a service cannot run a booking system or an emergency response on partial agreement.
Can the registration take the first fortnight of fees?
No, and it should not. Fees depend on the subsidy percentage, which is not known until the enrolment arrangement is confirmed. The form records the booking, the thank-you screen says what has to happen next, and the account is raised in the service billing system once the subsidy position is settled.
Why ask for a custody note when most families have nothing to write?
Because the families who do have something to write will not volunteer it, and the field is required with a write none instruction so the answer is deliberate either way. A court order that nobody at the service has seen is worth nothing at the gate, and the first time it matters is always the worst time to find out.
What file types can families upload for an action plan?
Images and PDF, which is the whole choice available on an upload field. That covers a scanned plan and a phone photo of the one on the fridge, and both are readable. The field allows up to three files at ten megabytes each, enough for a plan plus an updated page from the doctor.
