Childcare forms for centres, OSHC and holiday programs

A childcare service enrols children with detailed records, signs an agreement with families, records medication and excursion consent, and asks parents how the service is doing. This kit links finished examples of each, written for invented Australian services.

Enrolment records carry legal weight

A childcare enrolment is more than an application. Services in the national quality framework must keep enrolment records with specific information: the child details, parents and authorised nominees who may collect the child, emergency contacts, medical conditions and management plans, immunisation status, dietary requirements and the consents the service needs. The enrolment form collects these once, and the service keeps them up to date as children grow, change rooms and move on to school. An enrolment agreement then sets the fees, the days, the notice period and the policies the family agrees to.

Consents, programs and feedback

Day to day, a service records medication authorisations dose by dose, permission for excursions, and consent for photos. Outside school hours care and holiday programs need registration forms of their own, with bookings by day. Services also ask families for feedback, which is part of meeting the quality standard. The examples below cover enrolment, agreements, medication, programs and parent surveys as finished pieces, so a director can read every question before building their own form, and adapt the wording to their service and its policies.

Who the kit is for

The kit is written for long day care centres, family day care, outside school hours care and holiday programs in Australia. The National Quality Framework sets the record keeping and consent requirements, and families find services through the government Starting Blocks site, which links to the national registers. Every staff member needs a Working with Children Check. The services in the examples are invented, and each card opens the complete form or agreement rather than an outline, so every question can be checked against the service policies.

The kit, piece by piece

What the enrolment form must cover

The enrolment record needs the child name, date of birth, gender and address, each parent details, authorised nominees who can collect the child or authorise medication and excursions, emergency contacts, medical conditions with any management plan, allergies, immunisation status, dietary requirements and the service consents. Ask for Child Care Subsidy details where the family will claim it. The enrolment example in this kit groups these into sections so a family can finish it in one sitting.

Medication and excursions

Medication can only be given with written authorisation that names the child, the medication, the dose, the time and the method, and each dose given is recorded. Excursions need written permission that describes the excursion, the transport and the risks. Permission can be signed online by a parent; the e-sign flow confirms them with a code sent to their email and returns a certified PDF to the service.

Privacy and access to the forms

Enrolment forms hold sensitive information about children and families. Collect only what the service needs, tell families how it is used, and limit who can open the responses. A published form can require login or a secret key, and it does not email a copy of the answers to the parent; the service receives an alert on each submission and can export the records.

Questions people ask

What information does a childcare enrolment form need?

It needs the child details, parent details, authorised nominees, emergency contacts, medical conditions and management plans, allergies, immunisation status, dietary needs, days requested, Child Care Subsidy details and the service consents. The national regulations set the minimum records a service must keep.

Can parents sign childcare forms online?

Yes. Enrolment agreements, permission slips and many consents can be signed online. The parent confirms with a code sent to their email and the service receives a certified PDF with a record of when it was signed. Medication records still need the dose by dose log on the day.

How should a service record medication consent?

With a written authorisation from a parent or authorised nominee that names the child, the medication, dose, time, method and the dates it applies. Staff then record each dose given, the time and who gave and witnessed it. The medication consent example in this kit is laid out this way.

Do holiday programs need separate forms?

Usually yes, because they book by date and include excursions. A registration with a dated day picker, the excursion consents and the medical details for the program works best, linked to the main enrolment record where the child is already enrolled.

How often should childcare services survey parents?

Many services survey families once or twice a year and after a child first weeks. Ask about communication, safety, care and the program, and share what changed as a result. Family feedback is part of meeting the national quality standard.
