A permission slip e-signature gets signed where parents actually are, on a phone in the evening. This one page slip for a Year 5 museum excursion was signed on a 390 pixel wide screen, where the portal moves every typed field into a panel above the read-only page, and that phone journey is exactly what this page shows.

The signing journey
Step 1
Seven fields on one page
The school places two required emergency contact fields, an optional medical notes field, a required consent checkbox over the printed consent statement, the parent's name, a date and the signature. One envelope covers one child's slip.
Step 2
The phone gets a different portal
Below 560 pixels the document goes read-only and a Complete your details panel appears above it, telling the parent that everything filled in there lands on the page. No pinching around a PDF trying to tap a two millimetre box.
Step 3
Typing happens in the panel
Naomi types the emergency contact, the phone number, a note about a mild peanut allergy and the EpiPen in the school bag, and marks the consent box. The portal refuses to finish while any required field is empty.
Step 4
One finger draws the signature
The signature pad opens full width on the phone and supports touch, so the signature is drawn with a finger. Submission stamps the date, and the certified slip comes back to the parent and the school by email.
What the signed slip and certificate show
The completed page shows the typed contact details, the allergy note, the X in the consent box, the parent's name, the finger drawn signature and the stamped date. The certificate records one signer verified by email link, the consent gate's timestamp and version, and seven events from Envelope created to Signed, each hash chained to the last.
Running a whole class
One envelope covers one slip for one child, so a class of 26 is 26 envelopes sent from the same saved PDF, each parent holding a private link. The envelope list shows every slip's badge at a glance, and a slip still on Waiting two days before the excursion gets a resend, which rotates the link and appends a resent event. Separate envelopes also mean each family's medical note stays in its own certificate, not on a shared sheet.
How to adapt this envelope
Both parents signing means adding a second recipient. A camp form with more decisions adds more checkboxes, each one required and each printing its own X. If the school needs payment as well, that stays outside the envelope; this flow is the consent record, not the cash register.
What makes this signing flow work
Typed fields come to the parent, not the reverse
The compact portal's panel collects every input above the page with the exact promise that it lands on the page for you. The parent reads the slip full width and fills a form sized for a thumb, which is why these come back the same evening.
The consent box is required
The slip cannot be submitted with the consent statement unmarked, and the stamped page prints an X in the box beside the printed sentence naming the excursion, the date and the bus. The optional notes field, by contrast, could have been left empty without blocking anything.
The emergency contact is fresh, not archival
Schools hold enrolment contacts, but this field asks who to call on the day, typed by the parent that week. On the signed slip it sits beside the allergy note where supervising staff will actually look.
Questions people ask
Do parents need an account or an app?
No. The emailed link opens the portal in the phone's browser, and everything happens there: reading the slip, typing the details, drawing the signature. There is nothing to install and no password to invent at nine in the evening.
What actually changes on a small screen?
Below 560 pixels the document becomes read-only and every typed field moves into a Complete your details panel above it, with a note that whatever is filled in there lands on the page. The signature button sits in the same panel.
Can both parents sign one slip?
Yes, add the second parent as a second recipient with their own signature field before sending. For most excursions one parent's consent is what the school asks for, which keeps the envelope at one signer and the turnaround at one evening.
Can one envelope go to the whole class?
No. Each envelope is one document with its own signers, and each parent's link is private to them. Send one envelope per child from the same saved PDF; the envelope list then shows exactly which families have signed and which need a nudge.
Does the school get a copy?
Yes. The completion email carries the certified slip to the parent and to the sender, and the certificate of completion is a separate download from the envelope's status page for the school's records.
Can a parent decline instead of signing?
Yes, with a typed reason that goes to the school by email, and the envelope stops there. A child staying back is a legitimate answer, and a decline records it far better than a slip that never comes home.
