Why each excursion gets its own question
A single tick list of excursions looks tidier and collects worse data. Each of these three has a different price, a different departure time and a different hazard, and a parent saying yes to the cinema has not said yes to shallow water at the rock pools. Three separate questions also produce three clean columns in the export, which is what the coordinator prints and hands to the educator running that bus. The third option on each, not attending that day, keeps the answer honest for a family who ticked six days and is being asked about a seventh.
Sun, water and the pickup password
Three consents on this form are Australian in practice rather than in wording. Sunscreen is asked as two separate statements, because a family that wants their own product applied is agreeing to something different from one that wants an educator to apply the centre supply. Water consent is split the same way, with the rock pools and the centre sprinkler treated as different activities. The pickup password is the plainest safeguard on the page: educators ask for it whenever an unfamiliar adult arrives, and the paragraph above tells a parent to pick something a child can say out loud.
How to adapt this form
A council run program with rolling enrolments replaces the ten dated days with a two week block choice and adds a concession card field, since fees are means tested. Eighteen dollars and a bus at nine thirty are the two facts a parent needs to answer, so they sit in the label rather than in a paragraph above. A classic field description is not shown to the respondent, which makes the label the only honest place for them.
The pickup password is capped at forty characters
Short enough to stay a password and long enough for two words. The paragraph above asks for something a child can say out loud, which is the part that decides whether the safeguard survives contact with a shy eight year old at the door.

How many fields should a vacation care booking have?
Sixteen here, thirteen required. The day picker is one field doing the work of ten, which is why the form stays short despite covering a fortnight. Anything that belongs to the year rather than the fortnight, such as a court order or a subsidy reference number, lives on the annual enrolment instead.
Can this form take payment for the excursions?
