Age group is the question clubs get wrong
Community sport grades on the year a child was born, and families think in school years or in how old the child is today. The gap between those two produces most of the emails a registrar answers in February. Printing the birth year inside each option, Under 11 born 2016, removes the arithmetic entirely, and the paragraph above the question says the rule once in plain words. It also says what to do about the exception, playing up, which is a committee decision rather than something a parent can settle on a form at nine at night.
The Australian pieces on this form
The registration fee is stated as GST inclusive on the intro line, because a club invoicing a family should show the number that leaves the bank account. The concussion acknowledgement reflects the return to play protocols that Australian sporting bodies now publish for junior competitions, and it is written as something the parent accepts rather than something the club merely publishes. The thank-you screen raises the working with children check, since volunteer coaches recruited out of a registration round are the group most likely to be surprised by it.
How to adapt this form
A netball or basketball club keeps the age group block word for word and swaps position for a preferred division, since grading there runs on ability rather than on where someone stands. A cricket club adds a bowling and batting preference pair and a question about whether the player owns a helmet that meets the current standard. A club that runs both juniors and seniors adds a competition dropdown as the very first question and drops the guardian block for anyone choosing the senior competition.
What makes this form work
Every age group option prints the birth year it covers
Under 11, born 2016 answers the question before it is asked. A parent counting backwards from a birthday in November is the single most common source of a registration in the wrong grade, and the option text removes the count.
The medical question names the conditions it wants
Asthma, epilepsy, diabetes, a dislocating joint and a concussion in the last twelve months sit in the placeholder, so the box is not answered with the word fine. The separate inhaler and autoinjector question then turns the most urgent case into a sortable column.
Three codes are ticked separately
Player behaviour, sideline behaviour and concussion are three different promises, and a club that bundles them into one tick has collected one signature for three rules. Separated, the sideline statement is the one parents actually stop and read.

How many fields does a junior sport registration need?
Fifteen here, twelve of them required. A club that asks for less than this ends up chasing medical details in March, and a club that asks for much more loses the families who register on a phone during a school pickup. Anything about volunteering or fundraising belongs in a second form sent after the team sheets are out.
Why ask for a photo on a registration form?
Because the player card the referee checks at finals needs one, and collecting it in March is far easier than collecting it in August. The photo field takes a single image by design, and its description is one of the few field descriptions a classic form actually shows the respondent, which is why the framing instructions sit there.
What about consent when the player is a child?
The parent or guardian signs, and the three code of conduct statements are ticked separately so that nobody can claim they only read the first one. The concussion statement is deliberately last, because it is the one a club most needs a family to have actually seen before the first heavy collision of the season.
Can the registration collect the season fee?
No. It records the registration and the club raises the invoice afterwards, which matters because family discounts, hardship arrangements and any government active kids voucher all change the amount. A form that takes 265 dollars from a family entitled to a voucher creates a refund and a complaint on the same day.
Why is the emergency contact someone other than the parent?
Because away games are often an hour from home and the parent filling this form is usually the one in the car. The paragraph above the pair says so plainly. A contact list where every number rings the same phone is not a contact list, it is one number written out twice.
How does a club use the position answer?
For grading and for team balance, not as a promise. Six options including happy anywhere and has never played give a coach a starting picture without implying that a child who ticked goalkeeper will be in goal. Clubs that leave position as free text get answers like striker like Messi, which balances nothing.
