Sports registration form questions for a club season
Ask 15 to 20. Never skip the player's date of birth, medical conditions a coach must know about, and an emergency contact who answers on game day. Date of birth settles the age group, and the other two are what a coach reaches for when a player goes down.
A sports registration has to place each player in the right age group or grade, tell coaches what could go wrong on the field, and record who agreed to the codes, fees and photos. These questions work for juniors and seniors on one form, with the guardian questions clearly labelled for players under 18.

Which questions to make required
Nine questions are required: player name, date of birth, whether the player is under 18, age group, medical conditions, the emergency contact's name and mobile, the codes of conduct and the fee question. Guardian details are not required because senior players register themselves, so those questions are labelled for under 18s instead. Positions, uniform size and volunteering stay optional.

Three ways to change this bank
For representative tryouts, use the linked form's approach: add a nomination fee that is not refundable, first and second position choices, and the commitments that come with selection, and ask the player to sign as well as the guardian. For a school holiday sports clinic, replace the season and uniform questions with a grid of the days attended, whether lunch is ordered, and a table of people authorised to collect the child each afternoon. For a social adult league, remove the guardian section entirely, register the team rather than each player with a captain's contact and a roster table, and add a plain acknowledgement of the risks of playing, since seniors sign for themselves.
Questions people ask
Can one registration form cover juniors and seniors?
Yes, if the guardian questions are clearly labelled for players under 18 and not marked required. Ask whether the player is under 18 near the top, so the club can check that every junior has a guardian's details and consent. Senior players then skip the guardian section and sign for themselves.
How should age groups be written on the form?
As the birth years each group covers, not only as under 12 or under 14. Parents often guess the wrong group when they have to work out a cut off date, and a wrong age group means a regrade after teams are picked. Put the birth years inside each option so the choice is obvious.
What does child safeguarding mean for a registration form?
It shapes what the club collects and who sees it. Play by the Rules publishes guidance for clubs on keeping children safe, including policies, screening of adults in child related roles and careful handling of images, and the Australian Sports Commission names Sport Integrity Australia as responsible for child safeguarding in sport. On the form, that means clear photo consent, guardian contacts and medical information shared only with the people who need it.
Should the form ask for the player's gender?
Ask which competition the player is entering rather than asking for gender. The club needs to know whether a player is in an open, women's or mixed competition, and that question answers it directly. Anything more personal is not needed to register a player and should not be collected.
How can a club handle families who cannot pay the full fee at once?
Offer a payment plan answer next to the fee question, so asking for help is a normal choice rather than an awkward email. Many clubs also run hardship support or accept government sport vouchers where a state offers them. Say who to contact in the paragraph above the fee, and keep the answer private.
What should happen to medical information after registration?
Share it with the coach and team manager for that player's team, and nobody else. Keep it with the team's first aid kit or in a secure team list, and delete or archive it at the end of the season. A tournament entry has to tell the organiser which division a team belongs in, who is on the squad, and who is responsible for them on the day.
