Tournament entry form questions for team nominations
Ask 12 to 15 per team. Never skip the team name with its age group or division, the squad list with shirt numbers, and a manager who can be reached on tournament days. The draw needs the first, scorers need the second, and every problem on the day goes to the third.
A tournament entry has to tell the organiser which division a team belongs in, who is on the squad, and who is responsible for them on the day. These questions collect the team, its roster and its officials in one entry, so the draw, the scoresheets and the first aid list all come from the same answers.

Which questions to make required
Seven questions are required: team name, age group, the squad table, the manager's name and email, the fee question and the rules. Those are what the draw, the scoresheets and the tournament office cannot work without. Uniform colours, coach details, accommodation and travel constraints stay optional, since many teams settle them after entries close.

Three ways to change this bank
For a school sports carnival, enter by school rather than by team, replace the squad table with a table of students and the events each will contest, name the teacher in charge instead of a team manager, and leave medical consents to the school's own excursion forms. For a social corporate tournament, remove age groups, ask for the company and a team captain, add whether the team needs catering, and include a plain acknowledgement of the risks of playing, since every player is an adult. For a trivia or games tournament, keep the team and captain but swap shirt numbers for each player's handle, drop uniforms and divisions, and ask which session the team can attend.

What does a tournament entry ask that a season registration does not?
It registers a team for a few days rather than a player for a season. That shifts the questions to the division, the squad list with numbers, uniform colours for clash checks, accreditation passes, accommodation and travel constraints for the draw. Individual medical and guardian details usually stay with the club, which registered each player already.
Why collect shirt numbers on the entry?
Scorers and referees record goals, fouls and cards by number. Collecting numbers with the squad means scoresheets can be printed before the first game, and duplicates within a team are spotted in the office rather than on court. Ask for the number the player will actually wear, not a squad position.
Should teams choose their own division?
Let teams nominate, and say plainly that the organiser may regrade. Teams know their standard better than anyone, but some aim high for the challenge and some aim low to win. Describing each division's standard in the options, and regrading after the first round where the rules allow, keeps games competitive.
How can organisers avoid uniform clashes?
Ask for main and alternate colours, with shirts and shorts named separately, then sort the entries by colour before the draw is published. Teams told of a clash a week ahead can bring bibs or their alternate strip. Teams told at the court have to borrow, which delays the game and frustrates everyone.
How does the squad table come out in the export?
The response export is a CSV with one column per question, and a table question arrives as a single column holding the filled rows as a JSON list. For scoresheets, split that column into one row per player with a short script or a spreadsheet formula, then group the rows by team.
Should the entry form collect every player's medical details?
Usually not. The team manager holds that information and brings it to the tournament, and the entry only needs to flag that a player has a condition first aid should know about. Collecting full medical details for every player on a public entry form gathers sensitive information the organiser does not need to keep.
