Childcare enrolment questions for early learning centres

Ask most of them, because enrolment is not the place to cut corners. Never skip the immunisation statement, allergies with any medical action plan, and the list of people authorised to collect the child. A centre missing any of the three cannot safely take the child into care on day one.

A childcare enrolment has to tell educators who the child is, what could make them unwell, and exactly who may take them home, before the first day of care. These questions gather the health records, days and pickup authorities a centre relies on at the gate, and keep each consent separate so a family can agree to one and decline another.

Which questions to make required

Nine questions are required: the child's name and date of birth, the days wanted, the immunisation statement, the allergy question, a parent's name and mobile, the authorised collection table and consent to emergency treatment. Each is something educators need at the gate or in an emergency. Cultural practices, routines, the second parent and subsidy details stay optional, because families can add them at orientation.

Three ways to change this bank

For a kindergarten or preschool program, replace the days grid with the program's fixed sessions and term dates, ask whether the family is claiming a funded place at another service, and add consent to share a transition statement with the child's school. For family day care, add the educator the family has chosen, whether the child is comfortable around the pets at the educator's home, and consent for the educator to drive the child on outings. For outside school hours care, add the child's school and year level, turn the days grid into before school, after school and vacation care, and ask for consent for the child to walk between the school gate and the program with staff.

Questions people ask

Why are the days wanted asked as a grid?

Because families rarely want whole weeks. A grid with weekdays as rows and full day, morning and afternoon as columns shows the exact pattern each family needs, and a director can compare it against room capacity at a glance. A follow up question about taking other days turns a full room into a waitlist conversation rather than a refusal.

How should enrolment handle a child with anaphylaxis?

Ask the allergy question as required, then collect the ASCIA Action Plan completed by the child's doctor and any medication details before the first day. ASCIA plans are the standard action plans used throughout Australia and New Zealand. The form starts the process, but most centres also meet the family to agree how the plan works in the room.

What if the family's immunisation statement shows the child is not up to date?

In Victoria, No Jab No Play requires children to be up to date for their age to enrol, with catch up schedules and medical exemptions handled through the child's GP and the immunisation register. Once an exemption is recorded, the parent gives the centre the updated statement to confirm enrolment. Other states have their own rules.

Should an enrolment form ask about court orders?

Yes, as a plain yes or no, with a place to attach a copy. Educators cannot enforce an order they have never seen, and the pickup table alone does not show who is legally restricted. Keep the question neutral in tone, since families with an order in place are often already under strain.

Who should see the answers once they arrive?

The director and the educators in the child's room, and only the parts they need. Health details, pickup authorities and consents go to the room; subsidy numbers and court orders stay with administration. Export only the columns each group needs, since the response export puts every question in its own column.

Should the form ask about cultural and religious practices?

Yes, as an optional open question. Food, rest, clothing, celebrations and the names a family uses at home all shape how a child settles, and educators would rather know on day one than learn by accident. Leaving it optional respects families who prefer to talk about it at orientation.
