The temperament grid is the whole form
Most pet forms ask whether a dog is friendly, which collects a yes from everybody. This one asks seven separate situations and gives four answers to each, and the answers are graded rather than binary: relaxed, excitable but fine, wary or avoids, has reacted before. Food, bones and high value treats gets its own row because that is where most daycare incidents start, and toys with other dogs around gets another because the two are different behaviours. The paragraph above says plainly that a dog which has reacted is placed in a smaller group, not refused.
The Australian pieces on this form
C5 is the Australian shorthand for the core C3 vaccination plus the two kennel cough components, and it is what a daycare or boarding facility asks for. The upload note says a certificate from the vet is required rather than a photo of the tag, because the tag carries a date and no scope. Microchipping is compulsory in most Australian states and the number is fifteen digits, so the field enforces exactly that length. The signature paragraph names a 500 dollar limit for veterinary treatment without further contact, which is the local convention for an owner authority.
How to adapt this form
A boarding kennel adds arrival and departure dates, a bedding question and a second emergency contact, because a dog staying overnight has a different set of failure modes. A dog walking service drops the crate row from the grid, adds a key or lockbox question and a recall rating, since off lead work is the risk being assessed. A cat boarding facility replaces the whole grid with a litter, feeding and hiding preference set, and swaps C5 for F3, which is the equivalent feline cover.

Why is the dog photo required rather than optional?
Because it does two jobs. On the run sheet it lets a handler match a name to a dog at a busy drop off, which is when mistakes happen. If a dog ever gets out, it is the picture that goes out immediately, and a photo taken in the last six months is worth more than one from a puppy. The field takes exactly one image by design.
Can the form check the vaccination certificate itself?
No. It collects a PDF or a photo and the instructions say what will be rejected, which is a tag photo rather than a certificate, or cover that lapses before the trial day. A person checks it before the booking call, which the thank you screen states. Nothing on the form claims to read the document or contact the vet.
How does a rating grid work on a form?
It is a matrix field: seven statements down the side, four choices across the top, one answer per row. It exports as a single column with each statement and its answer paired, rather than seven columns, so read it as a block. Should a daycare ask about desexing?

What does the owner signature authorise?
The paragraph above it says the vaccination details are current and authorises veterinary treatment up to 500 dollars if the dog is injured and the owner cannot be reached, with the owner responsible for the cost. Naming the figure is the point. An open ended authority is the one owners dispute afterwards, and a limit gives the handler something to act on.
Why ask for feeding instructions as free text?
