Representative tryouts turn on two things a club usually finds out too late: what the player can safely do on the day, and whether the family has understood what a season actually costs. This registration asks both before anybody puts a bib on. It is a classic form, so a parent can see the health grid, the costs and both signatures on the one screen.

Health

Answer these for the player, not for yourself. A yes to any of them does not affect selection. It tells the trainer what to have on the sideline and the selectors what not to ask a player to do on the day.

A player returning from a concussion needs written clearance from a doctor before the first trial, not before selection. Bring the paper copy with you. We cannot accept it over the phone on the morning, and a player without it will be asked to watch.

What nominating commits you to

The nomination fee is $30 and it is not refundable. If the player is selected, 2027 levies are between $780 and $1,050 depending on the division, and the playing uniform is bought outright on top of that. We would rather you knew all of it now than in March.

What makes this form work

The cost of selection is a tick box, not a footnote

Five statements covering the non refundable nomination, the levy range, the uniform, attendance at both sessions, and the eighteen month penalty for walking away after selection. Families read them because they have to tick them, and a club stops losing players in March.

A five row grid replaces a page of medical questions

Conditions, injuries, medication, concussion history and current clearance sit in one block with a yes and a no column. It takes fifteen seconds, and the single follow up box below catches the detail that actually matters from the handful of families it applies to.

Two signatures, two different promises

The parent consents to treatment and accepts the costs. The player commits to turning up and to the code of conduct. Splitting them is the difference between a consent form and an agreement, and it costs one extra box on the screen.

Questions people ask

What does a tryout registration need that a season registration does not?

Position preferences, a recent grading history the selectors can read, and the health picture for one afternoon rather than a whole season. It also needs the money conversation, because a family that discovers the levy in March is a family that withdraws in March and leaves a squad short.

Why use a grid for the health questions?

Five yes or no rows in one grid read faster than five separate questions and are answered more honestly, because nobody feels singled out by a page of medical boxes. One follow up box underneath collects the detail from whoever answered yes, which is usually two families out of forty.

Should the player sign as well as the parent?

For a junior, yes. The parent consents and accepts the costs, and the player signs to say they will turn up to both sessions and behave. Two separate signature boxes make that difference visible, and a fourteen year old signing their own commitment takes it more seriously than one their parent signed for them.

How do I handle a player coming back from concussion?

Ask about the last twelve months, then say plainly that written clearance from a doctor must arrive before the first session and be carried on paper. The point of the question is not to exclude anybody, it is to make sure the letter is in a bag rather than in an email nobody can open courtside.

One form, one team, and the tournament office needs all of it before it can draw a pool. This entry takes the division nomination and the evidence behind it, fourteen players in a table, two adults reachable on the weekend, and the child safety checks that decide who is allowed on the bench. It is a classic form, because a squad table and a fee ladder do not survive being broken into single questions.

Reimbursement request form

A community club runs on people who buy things with their own money and then feel awkward asking for it back. This request makes asking straightforward and gives the treasurer the four things a set of club books needs: the budget line, the approver, the receipt and a payment method. It is a classic form, because a photo field cannot be filled in on a conversational one, and the receipt is the whole point.

Fun run entry form

A community fun run entry that has to do three jobs at once: sell a place, put the right person in the right wave, and hold a waiver that stands up. Every field here exists because a race director has been caught out by not having it at half past six on a Sunday morning. It is a classic form, because a waiver has to sit visibly above the signature that accepts it.
