Band booking enquiry form with the line up and the stage

A band booking enquiry form has to match three things: the line up the client wants, the space and power the venue has, and the budget. This one gives each line up its stage size in metres, asks what the venue offers, from separate power circuits to a sound limiter, and takes a budget band so the quote is pitched right.

Book Velvet Sound

Velvet Sound is a soul, pop and Motown covers band. We play as a duo, a trio or the full five piece, with our own PA for up to 200 guests. Tell us about the night and we will tell you which line up fits.

The event

The band

The five piece needs two separate 10 amp power circuits close to the stage, and drums need a solid floor. Many venues have a noise limit or a sound limiter, so tick it if you know there is one.

What each field on this form collects

Type of event is a dropdown of six beside the date. Venue is an address block. Line up is a radio of four: a duo needing 2 by 2 metres, a trio needing 3 by 2, the five piece needing 5 by 3, or not sure. How much live music is a radio of four set plans, from one 60 minute set to ceremony music plus two reception sets. A paragraph explains the two power circuits and noise limits, then at the venue is a required checkbox of six, from a stage to stairs only load in. Budget is a radio of five bands including a quote first option. First dance song is optional short text. Your name and a mobile and email line close it.

Why the stage size is in the line up label

Couples choose the five piece from a video and the venue gives them a corner by the bar. Writing 5 by 3 metres into the option means the person booking can look at the room plan before choosing, and the not sure option invites the band to recommend instead. The venue checklist then confirms the other half: whether there is a stage, two power points on separate circuits and room undercover if the event is outside. Together they stop the most common band booking failure, the full band arriving to a space that fits a duo.

How to adapt this form

A string quartet changes the line up to solo, duo, trio and quartet, swaps power for chairs without arms and shade outdoors, and adds a ceremony music list. A jazz trio for corporate events asks for background or feature music and the volume the client expects during speeches. A tribute act adds the costume changes and a dressing room question, and replaces the first dance with the opening song the audience will know.

What makes this form work

Each line up states its footprint

Duo 2 by 2 metres, trio 3 by 2, five piece 5 by 3. The person enquiring can check the room before choosing, and a not sure option turns a guess into a recommendation from the band.

The venue checklist covers power and noise

Two separate circuits, cover outdoors, a sound limiter, stairs only load in. Each is a known reason a band booking fails on the night, and each is answered before the band quotes rather than discovered at load in.

Sets are planned, not counted

Four real set plans, from one hour to ceremony music plus two reception sets, with a playlist between sets where that applies. The answer maps straight onto the quote and the timeline, instead of a number of hours the band has to interpret.

Questions people ask

Why does a band booking form ask for a budget?

Because the same Saturday could be a duo for $1,500 or the five piece with ceremony music for $4,000 or more. A budget band lets the first quote fit instead of starting a negotiation. The last option, I need a quote to decide, keeps the question honest for people who have no figure yet.

What does a live band need from the venue?

Floor space to match the line up, power on separate circuits so the amplifiers do not trip the lights, cover if the event is outdoors, and an easy load in. The checklist asks each of these, plus whether the venue has a noise limit, which can rule out a full drum kit altogether.

What is a sound limiter and why does it matter?

It is a device some venues fit that cuts the power when the volume passes a set level, often to protect neighbours. A drum kit can trigger one on its own. If the box is ticked, the band may suggest an electronic kit or the trio, which is why the question is asked before the quote.

Why ask for the first dance song on an enquiry?

Because the band may need to learn it, and a song outside its usual set takes rehearsal time and sometimes a fee. Knowing it at the enquiry stage means the quote can include it. The field is optional, since corporate events and parties have no first dance.

Does sending the enquiry book the band?

No. The thank-you screen says a quote and a sample set list arrive within two days, and the date is held for seven days once quoted. A form records the enquiry and does not check the band calendar, so the booking is confirmed by the band in writing, usually with a deposit.

How early should a live band be booked for a wedding?

Saturday dates in the busy season are often taken nine to twelve months ahead, and popular bands book earlier still. The enquiry asks the date in the first row so the band can answer the most important question first. If the date is gone, the quote can suggest a Friday or a Sunday, which are usually cheaper.
