Made to measure order form with a measurements table
A made to measure order form has to collect measurements precise enough to cut from, with the method stated, because a blind cut too wide cannot be fixed. This one explains inside and outside mount before a six row windows table of room, width, drop and mount in millimetres, and asks for a photo of each window.

What each field on this form collects
Product is a radio of four, two rollers, a roman and a timber venetian, all cordless or motorised, beside fabric or finish, a dropdown of eight. A paragraph explains inside and outside mount and how to take the smallest recess width. Windows is a required, numbered six row table with room, width in millimetres, drop in millimetres, and inside or outside mount. Control side is a radio of three, beside an optional upload of up to ten measurement photos. Installation is a radio of two, with the fitting price. The address, name, email and phone close the form. Nine of the ten fields are required.
Why the measuring method comes before the table
Most made to measure mistakes come from the method, not the tape. An inside mount blind needs the recess width at its narrowest point, and the maker deducts a small clearance; an outside mount blind needs the finished size the customer wants. Mix them up and the blind either jams in the recess or leaves a light gap. Writing the method in a paragraph directly above the table, and putting mount in its own column, means each row is read the right way. The photo of each window with the tape in shot lets the workshop catch a width written as 1200 that is really 1020.
How to adapt this form
A curtain maker swaps drop for floor to rail height and adds a heading style column. A shutter company adds frame type and a column for the number of panels. A flyscreen maker adds window or door and the frame colour, and drops fabric. A business that also measures on site adds a radio offering a paid measure and quote visit, credited against the order.
What makes this form work
Method first, numbers second
The paragraph on inside and outside mount sits right above the table, and mount has its own column. Every width is read with the right clearance rule.
Everything in millimetres
Width mm and drop mm are the column headings and the intro asks for a steel tape. The workshop cuts straight from the numbers without converting or guessing at rounding.
A photo backs each window
Up to ten measurement photos with the tape across the recess. A transposed digit is caught when the photo and the row disagree, before the fabric is cut.
Questions people ask
Should blinds be measured in millimetres?
Yes. A few millimetres decide whether an inside mount blind fits the recess, and centimetres invite rounding. The table columns say mm and the intro asks for a steel tape, which does not stretch. Measuring each window three times across and giving the smallest width is standard practice for an inside fit.
What is the difference between inside and outside mount?
Inside mount sits within the window recess for a neat, built in look, and needs the recess deep enough for the bracket. Outside mount fixes above and over the recess, blocks more light at the edges and suits shallow recesses. The paragraph on the form explains both and how each is measured.
Why are all the products cordless or motorised?
Because looped cords on blinds are a known strangulation risk for young children, and window covering safety bodies recommend cordless products in homes with children. Offering only cordless spring, wand tilt and motorised controls removes the risk from the order rather than leaving it to a cord tensioner.
Can made to measure blinds be returned?
Not for a wrong size, because they are cut to that window. The intro says so, and it is the reason the form asks for a photo of every window with the tape in shot. The workshop compares each row with its photo and confirms every size in the quote before cutting.
What if the house has more than six windows?
The table shows six rows and cannot grow, which covers most single room or apartment orders. A whole house order goes in as two submissions, such as upstairs and downstairs, or the customer books a measure and quote visit. Numbered rows keep each window easy to refer to in the quote.
What does control side mean?
The side the chain, spring handle or wand sits on, seen from inside the room. Near a wall or a bed head it matters which side is reachable. The form offers right for all, left for all, or mixed with the side written after the room name in the table.

Firewood order form with delivery access and stacking
A firewood order form has to settle what wood, how much, and whether a tipper truck can get to the spot where the wood will be burnt or stored. This one prices four quantities by the tonne, asks about driveway width, low branches and wires, and offers stacking as a priced extra.
Spare parts order form with model, serial and part numbers
A spare parts order form has to identify the exact machine before the part, because the same model can take different parts across production runs. This one asks the model and serial number first, takes part numbers, descriptions and quantities in a numbered table, and accepts a photo when a part number is missing.
Custom furniture order form with dimensions, timber and finish
A custom furniture order form has to capture exact dimensions, the timber and the finish, plus how the finished piece gets through the door, because a made to measure table cannot be returned. This one takes the size as one line of three numbers, asks about stairs and hallways, and records a signed deposit agreement.
