Order form templates, field by field

An order form has to capture exactly what was ordered, in what quantity and size, and how it reaches the buyer, so nobody has to ring back to ask. Each form here is finished and readable in full, with the questions that kind of order cannot do without.

The detail that makes the order buildable

Twenty four forms, and each one turns on the detail a maker needs before starting work. The t-shirt form takes sizes and quantities in a grid and the print position. The engraving form caps each line at thirty characters and asks for a font and layout, because a wrong letter cannot be undone. The custom furniture form takes width, depth and height in one line and asks how a large piece gets into the house. The firewood form asks whether a tipper truck can reach the drop point. The spare parts form asks for the machine model, the serial number and a photo of the worn part. The school lunch form asks about allergies with a short note on why, and takes a parent signature. Start from the order closest to yours and keep its deciding question.

Grids, uploads and how quantities are taken

Thirteen of the forms take quantities in a table with named columns. A table shows a fixed number of rows and a buyer cannot add more, so each form sets enough rows for a normal order and says what to do with a bigger one. Artwork, logos, reference photos and guest lists come in through a file upload, which accepts images and PDFs. Where a rule limits a choice, such as four balloon colours or two swaps in a veggie box, the form states the rule in words, because a checkbox list does not stop a fifth tick. Four of the forms are conversational, one question to a screen, which suits a short repeat order on a phone: coffee beans, a bakery pre order, flowers and a bulk enquiry. The other twenty are classic forms on one page.

What an order form does not do

None of these forms adds up a total or takes a payment. A submission is a request, and a person replies with the price, a proof or a payment link, which is what each form tells the buyer. Prices and deposit terms are written into the form as text so they are read before ordering. Eight of the forms are Australian and use Australian terms and GST inclusive prices. An owner can turn on an email alert or a webhook so each order reaches the inbox or the system that fulfils it.

Start from a description instead

You do not have to start from an example. Describe the form you need in a sentence and the generator writes the fields for you, in your language and your colours.

Build a form

Step by step in the builder: the components an order form uses, then send each order to an inbox or a webhook. For everything the generator can do, see the form maker.
