Engraving order form with a text proof and font choice
An engraving order form has to get the exact words, spelled and spaced as they should be cut, because an engraved mistake cannot be fixed. This one limits each of two lines to 30 characters, asks the font and layout as named styles, and sends a proof for approval before any item is engraved.

What each field on this form collects
Item is a dropdown of seven, from a stainless steel bottle to a trophy plate, beside a quantity from 1 to 200. A paragraph explains the 30 character limit and asks for the text exactly as it should appear. Line 1 is required and line 2 optional, each a short text with a maximum length of 30. Font is a radio of four named styles, serif, sans serif, script and block capitals, beside layout, a radio of four including curved around a logo and text only. The logo upload is optional and takes one PDF or image. Needed by is a date, then name and an email for the proof. Eight of the ten fields are required.
Why the character limit is stated twice
Every engravable surface has a width, and 30 characters is what fits a bottle or a glass at a readable size. The limit is in each line label and in the paragraph above, because the form refuses a longer line only at submit, with a general error rather than a message on the line. Saying it up front means the customer shortens "Clara Hernandez and Tomas Okonkwo" while they are thinking about it. The proof then shows the chosen font at real size, which is where a customer spots that a script font makes a long name hard to read.
How to adapt this form
A trophy engraver turns the two lines into a table of plates, with name, award and year per row, since a club orders twenty at once. A jeweller adds ring size and an inside or outside choice and drops the logo. A corporate gifting shop adds a question asking whether each item carries a different name, with an upload for the list as a PDF.
What makes this form work
The text limit is enforced and explained
Each line carries a maximum length of 30 and says so in its label, and the paragraph above repeats it. The words that reach the proof already fit the item.
Fonts are named by look
Classic serif, clean sans serif, script and block capitals, each with a short description. Customers choose a style they can picture, and the proof shows it at real size.
Nothing is cut before approval
The intro, the submit button and the thank-you screen all say a proof comes first. A form for a permanent product makes the approval step impossible to miss.
Questions people ask
How many characters fit on an engraved item?
It depends on the surface. A pen holds 15 to 20, a bottle or a whisky glass around 30 per line at a readable size, a brass plaque far more. This form sets 30 per line because most of its items are bottles and glasses. A shop with larger plaques raises the limit on those.
Why does an engraving order need a proof?
Because the cut is permanent and the most common mistake is a name spelled the way the customer heard it. A proof shows the text in the chosen font at real size, so letters, spacing and line breaks are checked by the person who knows the names. The thank-you screen asks the customer to read every letter.
Can a full colour logo be engraved?
Not in colour. Engraving marks the surface in one tone, so the upload description asks for a black on white file and says gradients become solid lines in the proof. A detailed photo logo usually needs simplifying, and the proof is where the customer sees whether the simplified version still reads as their mark.
What happens if a line is longer than 30 characters?
The form refuses the submission with a general error message, not a note on the line itself. That is why the paragraph above the text and both labels state the limit. Counting spaces surprises people, so the placeholder shows a typical line that fits comfortably inside the limit.
Why is the second line optional?
Many orders are a single name or a single word, and an empty second line keeps the layout centred. Making it required would push people to fill it with something they did not want engraved. The layout question includes text only and centred options, so a one line order still has a defined layout.
How long does engraving take after the proof?
Three business days for up to 20 items, as the thank-you screen says, counted from approval rather than from the order. The needed by date lets the shop see straight away whether a slow proof approval puts an anniversary or a presentation at risk, and call the customer before it does.

T shirt order form with a size grid and print position
A t shirt order form has to collect how many of each size, which colour and where the print goes, because those three set the price and the screens. This one uses a six row size and quantity table, gives each print position its maximum width in centimetres, and takes the artwork as an upload with the file rules stated.
Meal prep order form with a weekly menu grid
A meal prep order form has to turn a weekly menu into a cooking list: which meals, how many of each, what size and which delivery day. This one prints the week's menu above a seven row meal and quantity table, asks portion size and dietary swaps, and is set up to send each order to the kitchen sheet by webhook.
Custom furniture order form with dimensions, timber and finish
A custom furniture order form has to capture exact dimensions, the timber and the finish, plus how the finished piece gets through the door, because a made to measure table cannot be returned. This one takes the size as one line of three numbers, asks about stairs and hallways, and records a signed deposit agreement.
