Product sample request form with project details

A product sample request form has to say which samples to send and where, and give the supplier enough about the project to know which requests are a future order. This one lets a visitor pick up to five tiles, then asks the project type, its stage, the area in square metres and whether they are in the trade.

Request tile samples

Kilnstone Tiles. Free cut samples, posted within two business days. A sample shows colour, surface and edge in your own light, which no screen can. Choose up to five, tell us a little about the project, and the samples come with the price per square metre for each.

Samples

If you tick more than five, the first five in the list are sent. Each sample is a cut piece about 100 x 100 mm, or a whole tile for the small formats.

What each field on this form collects

Which tiles is a required checkbox of ten in two columns, each option giving the range, colour and size in millimetres, labelled choose up to five. A paragraph says only the first five are sent and describes the sample size. Project type is a radio of five, beside project stage, a radio of four from just ideas to tiling within a month. Square metres is an optional number, beside a required radio asking homeowner, tiler or builder, or architect or designer. The postal address, the name and email are required; the phone is optional. Seven of the nine fields are required.

Why the form asks about the project at all

A free sample costs the supplier the tile, the cutting and the postage, and most requests never become an order. Project stage and square metres let the supplier see which requests matter this month: a builder booked with 60 square metres is a different conversation from just ideas. The trade question routes requests to the right person, since tilers and designers often get trade pricing. Turned on in the add-ons, the email alert sends each submission to one address with the answers formatted in the message, so the consultant sees the stage and area without opening the dashboard.

How to adapt this form

A flooring supplier swaps tile sizes for board widths and adds underfloor heating. A paint company sends colour swatches and drops square metres for the number of rooms. A fabric house adds a commercial use question, since rub count matters for contract upholstery. A stone supplier adds slab or offcut, because a sample of natural stone shows only part of the veining in a full slab.

What makes this form work

Each option names the size

Ten tiles, each with range, colour and size in millimetres. A visitor comparing a 100 x 100 zellige with a 600 x 1200 porcelain sees the scale difference before the samples arrive.

The limit says what happens next

Choose up to five, and the paragraph says only the first five ticked are sent. Requests stay within budget without pretending the checkbox blocks a sixth tick.

Stage and area rank the leads

Project stage from just ideas to tiling within a month, plus square metres and trade, tell the consultant which of the day's requests to follow up first.

Questions people ask

Why limit tile samples to five?

Because each sample costs the supplier, and more than five rarely helps a decision. The label says up to five and the paragraph explains what happens with a sixth tick: only the first five in the list are sent. A checkbox question cannot block extra ticks, so the rule is written down and applied the same way each time.

Should a sample request form ask for a phone number?

Optionally. Many people requesting samples do not want a sales call, and a required phone field drops requests. This form asks for email, which is enough to send tracking and prices, and makes the phone optional. The trade question tells the supplier when a call is likely to be welcome.

How does the supplier hear about a new request?

With the email alert add-on turned on, each submission goes to one address with the submitted values in the message. Only the owner gets that email; the person requesting samples gets no automatic reply from the form. The thank-you screen tells them when the samples are posted instead.

What does project stage tell a tile supplier?

How soon an order could come. Just ideas is months away, tiling within a month is urgent, and a sample that arrives after the tiler starts is wasted. Suppliers often post urgent requests by express or offer a showroom visit. The four stages are written in the customer's words rather than sales terms.

Why ask for square metres when ordering a sample?

It sizes the job. A 4 square metre splashback and a 120 square metre floor need different conversations about stock, batch matching and delivery. The number is optional, because many people do not know it yet, and its placeholder gives a typical bathroom figure to show the scale expected.

Can a visitor request samples from outside the delivery area?

The form accepts any address, so the supplier decides. A supplier posting nationally sends anywhere. One with a local service area adds a line to the intro naming it, or replies to out of area requests with a nearby stockist. The postal address field is required so every sample has somewhere to go.
