Balloon order form with colours, garland length and setup
A balloon order form has to capture the product, the colours, the size in metres and whether the decorator installs on site, because those four set the balloon count and the time on the day. This one gives a length guide by wall and table, limits colours to four in the label, and asks for a photo of the space.

What each field on this form collects
Product is a radio of six in two columns, from an organic garland to ceiling balloons. Colours is a required checkbox of twelve in three columns, labelled choose up to four. Garland or arch length takes metres from 1 to 30, beside optional text on the balloons limited to 40 characters. A paragraph gives the length guide by space. Inspiration is an optional upload of up to three photos. Event date sits beside setup or pickup. The venue address is optional, since a pickup order has no venue. A safety paragraph covers foil balloons. One short text takes the name, mobile and email. Five of the nine fields are required.
Why the length guide is in metres by space
Customers rarely know how long a garland should be, but they know where it goes. A cake table, a feature wall, a bar or a wide doorway each map to a length, and the length sets how many balloons are inflated, which is most of the price and the setup time. The paragraph turns a place into metres, so the quote starts close to right. Colours have the same problem in reverse: customers want everything. The label says up to four, because a garland with more than four colours reads as random; the checkbox cannot stop a fifth tick, so the mock up settles it.
How to adapt this form
A studio doing corporate work adds brand colour codes and a logo upload for printed balloons. A party hire business adds backdrop and plinth hire as checkboxes. A studio delivering helium bouquets only drops length and venue and adds a delivery window and recipient name.
What makes this form work
Length is tied to the space
The guide says 2 m frames a cake table, 3 to 4 m covers a wall, 6 m runs along a bar. Customers who cannot picture metres can still give a length the quote can use.
Colours come with a limit
Twelve colours in three columns, labelled choose up to four. The palette arrives already edited, and the colour mock up only fine tunes it.
Safety is written into the order
A paragraph explains why foil balloons stay indoors or weighted. The customer reads it before the event, not from the installer on the day.
Questions people ask
How many balloons are in a metre of garland?
Commonly 25 to 40 for an organic garland, depending on how full it is and the mix of balloon sizes. That is why the form asks for length in metres rather than a balloon count: the decorator converts length into balloons and price. The paragraph on the form maps lengths to spaces so customers can estimate.
Why limit colours to four?
Because a garland with five or six colours loses its pattern and reads as a mixed bag. Four, including a neutral, is enough for a designed look. The label says up to four, and the checkbox cannot block a fifth, so the colour mock up after the quote is where the final palette is agreed.
Are foil balloons safe outdoors?
They are safe when tied down or weighted, but a loose foil balloon is a hazard near power lines, because the metallic coating conducts electricity. Power companies report outages caused by foil balloons every year. The paragraph above the contact field says foil balloons are kept indoors or weighted, which the installer follows.
How far ahead should balloons be booked?
For weekend events, about three weeks, as the thank-you screen says, and longer in wedding and graduation seasons. The date is held only once the deposit is paid, which stops a date being double booked by two quotes. A midweek corporate event can often be booked with a week of notice.
Why is the venue address optional?
Because a pickup order has no venue for the studio to visit. The setup or pickup question decides whether the address matters. When a customer chooses installation and leaves the address blank, the studio asks for it with the quote. A required address would force pickup customers to type their own home.
Can the text on a bubble balloon be any length?
No. The short text field allows up to 40 characters, and a longer entry is refused at submit. Vinyl lettering on a clear bubble balloon has to fit the curve and stay readable from across a room, so a name and an age, or two short words, work best.

Invitation order form with wording, quantity and proofs
An invitation order form has to collect the exact wording, spelled as it will print, plus the count, the envelopes and how many proofs it takes to get it right. This one separates the names and the date line from the full wording, explains household counts, and takes the guest list as a PDF for addressing.
Gift hamper order form with a recipients table and gift messages
A gift hamper order form has to send many hampers from one buyer to many people, each with their own address and card message, on one dispatch date. This one puts recipients in a numbered six row table with name, delivery address and message columns, and takes the invoice details with an ABN.
Coffee bean subscription order form with grind and cadence
A coffee bean subscription order form has to match the grind to how the person brews and the delivery gap to how fast they drink, or the beans arrive stale or too late. This one asks both on their own screens, explains each choice in grams and cups, and sets the first roast date before taking the address.
