Real forms, field by field
Every form here is finished and readable in full. The field lists come from how these forms are actually written in the trade, not from a template gallery, and each page says why a field is there before you copy it.

Market stall application form with insurance and power on it
A market coordinator is allocating fifty sites and balancing a row, which means the useful answers are the boring ones: what you sell, how big your marquee is, whether you need power and whether your insurance outlasts the season. This fourteen field form collects all four and gets the certificate expiry as a sortable date.

Martial arts registration form with grade, injuries and waiver
A dojo has to know two things before somebody steps on the mat: what they can already do and what their body cannot take. This registration asks both, then treats sparring as a separate consent rather than as something a membership implies, so a new member can start training on the first night without agreeing to be hit.

Massage booking form for a mobile therapist
A mobile massage booking is a logistics problem before it is a wellbeing one, because somebody has to carry a table from a car to a room. This booking form settles where the session happens, what the parking is like and how long it runs, then asks about style and pressure in words a client can actually answer.

Massage consent form with draping, areas and the right to stop
A massage consent form records which treatment a client agrees to, which parts of the body may be massaged and that they can stop at any time. This one also explains draping in plain words, asks about pregnancy and health conditions, and takes the client's signature before the first appointment.

Massage intake form that asks about pressure before the table
A client lying face down with their eyes closed will almost never ask for lighter pressure or say that they would rather their feet were left alone. This fourteen field intake form asks both in advance, alongside a contraindication screen that adapts the technique rather than cancelling the booking.

Meal prep order form with a weekly menu grid
A meal prep order form has to turn a weekly menu into a cooking list: which meals, how many of each, what size and which delivery day. This one prints the week's menu above a seven row meal and quantity table, asks portion size and dietary swaps, and is set up to send each order to the kitchen sheet by webhook.

Media release form for an interview, recording and attribution
A media release form is the written permission an organisation needs before it records someone and publishes their words or image. This one lets a person interviewed by a foodbank choose what is recorded, how they are named, where the story appears and whether they see it first.

Medication consent form for childcare, dose by dose
A medication consent form for childcare has to authorise educators to give a named medicine at a set dose, time and route, and show when the child last had it. This one puts those four details in a table copied from the pharmacy label, then asks the reason, prescriber, expiry, storage and last dose before a guardian signs.

Meeting room booking form with catering and AV in one request
A meeting room request that arrives without a layout, a headcount and a cost centre becomes three emails and a coordinator moving chairs while eight people wait in a corridor. This booking form asks for all three in ten fields, puts the seated capacity beside every room name, and states the notice period on each catering option.

Mentorship program application form for mentors and mentees
Running two separate forms for mentors and mentees means two sets of answers that cannot be compared, which is the hardest part of matching. This thirteen screen form asks both sides the same questions, including what they want out of it, and asks for no more than three ticks on the expectations question so a match has something specific to be built on.

Midwifery intake form for a private booking visit
A booking visit with a private midwife is ninety minutes and should mostly be about what the woman wants, not about reconstructing her history out loud. This twenty field intake form takes the due date, the previous pregnancies, the blood group and the birth preferences beforehand, and asks who will be in the room.

Mortgage broker fact find a lender can read in one sitting
A fact find is the document a loan application is actually built from, and it falls apart when income, debts and living expenses arrive in three emails a week apart. This one takes all three as tables on a single form, with the loan purpose at the top and the credit consent signed at the bottom.

Music lessons registration form that matches a teacher
A teaching studio is really a matching problem, because the instrument is only one of four things that decide who should teach a new student. This eleven screen registration collects all four, and ends with an optional recording so a teacher can hear a returning player rather than guess from a grade they gave themselves.

Nail appointment booking form that asks about removal first
A nail appointment overruns for one reason more than any other, which is that the previous set had to come off and nobody booked time for it. This booking form asks about removal on the second screen, before the date is chosen, and takes the saved design as an upload so the technician can say whether it fits the slot.

Naturopath intake form with an eight system review grid
A naturopathic consultation asks about eight body systems and everything already being swallowed, and doing that inventory out loud costs most of a first appointment. This nineteen field intake form takes the grid and the supplements table beforehand, so the ninety minutes go on the conversation instead.

NDIS intake form that starts from the plan itself
An NDIS provider cannot start a support without five things off the plan: the participant number, the two dates, the management type and the goals. This nineteen field intake form asks for all five, plus the participant name as the plan shows it, and tells them to have the plan open while they answer.

New employee onboarding form
A new employee onboarding form collects everything payroll, IT and the manager need before a new starter's first day: legal name, contact details, right to work, bank and super details, the role, equipment and policy acknowledgements. This one is the single pre start collection for an Australian employer, ordered by who needs each answer, so the new starter types their address once instead of three times. It is a classic form, because people and culture reads the whole response in one pass and forwards each block to the team that owns it.

New hire onboarding survey at day 90
A new hire onboarding survey asks recent starters what helped and what was missing in their first months, early enough that the answers can fix the process for the next intake. This day 90 version covers role clarity, day one readiness, the buddy, a training grid, manager check ins and whether the person feels part of the team.

New patient registration form with Medicare and IRN on it
A new patient form has to produce a file a doctor can safely open, which means a Medicare number with the right IRN, a medicines list that includes the things people forget, and consents that were actually read. This one asks nineteen questions and explains the two that people get wrong most often.

New vet client form with the pet record and the consent together
A vet's first consult is shorter when the microchip number, the last vaccination and the previous clinic's name are already on file. This form collects the owner, one pet and the consent a clinic needs, and asks for the previous vet so the records arrive before the appointment does.

Newsletter reader survey on topics, frequency and format
A newsletter reader survey asks subscribers what they want more of, how often they want it and how they read it, so the next issues fit their week. This one does it in seven screens for a weekly money newsletter, ending with a single question on interest in a paid tier.

Newsletter signup form
One field is the correct length for a newsletter signup, and everything beyond it has to earn its place by changing what gets sent. This one asks six questions: the address, an optional name, what somebody wants to hear about, how often, how they buy, and a consent line that starts unticked and names the sender. It is conversational, because six one line questions on six screens is faster on a phone than a block of them on one.

NPS survey with the score explained
Net Promoter Score is one question, and most surveys ruin it by adding nine more. This one is the question, one follow up, and a permission to reply, and the page shows how the number is worked out from the responses table.

Occupational therapy intake form built on a daily living grid
An occupational therapist is asked to change how a day works, so the intake has to describe the day rather than the diagnosis. This eighteen field intake form takes an eight task support grid, the equipment already in the house, and the funding with its plan dates, before the first home visit is booked.
