The student number does two jobs
It proves somebody is enrolled, which is usually the condition of a society grant, and it is the key the committee reconciles against the enrolment roll each term so the mailing list does not fill with graduated members. The field only accepts eight digits, which catches the most common error, a campus card number typed with its spaces. The paragraph above it says where the number appears on the card, because half the people filling this in have been enrolled for three weeks and have never had to quote it.
The interest list is a planning document
Ten activities in two columns, with a paragraph saying to tick anything you would come to, including things you have never done. That instruction turns a preference question into the society programme, because a committee that sees eight ticks against caving in March goes and finds somebody who can lead a caving trip. Two of the ten are training rather than trips, leader training and first aid, and those are the ticks that tell a committee who might run the club in two years.
How to adapt this form
A society that charges different rates keeps everything and adds a membership type question, since concession and postgraduate rates are the ones people ask about. A club with a safety qualification requirement adds a certificate upload limited to images and PDF, because a photo of a card is what members actually have. A faculty society replaces the trips list with an events and committee interest list, and adds a degree programme field, since the entire value of that mailing list is being able to write to second year law students specifically.

The society page and union recruitment publications are separate ticks, and the paragraph says a blank changes nothing about trips. Consent collected that way is worth something, where a bundled tick is worth very little.

How many fields should a club membership form have?
Nine here, seven required, and the intro line says only the first five make somebody a member. That framing matters on a form handed out at a stall during orientation week, where the realistic attention span is about ninety seconds and a queue is forming behind the laptop.
Why restrict the student number to eight digits?
Because the common mistakes are a campus card number with spaces and a staff number of a different length. The pattern catches both at the point of entry. It cannot confirm the number belongs to a real enrolment, which is why the committee reconciles the list against the roll once a term instead.
Can the form take the membership fee?
