HR form templates for every stage of employment
An HR form collects the details and decisions that move a person through employment, from referral and interview to leave, review and resignation, in a way that is fair, consistent and on the record. Each form here is finished and readable in full, with the reason each field is there.

Joining
Six forms cover getting someone in the door and paid. An employee referral form asks whether the candidate agreed to be referred. A conversational interview availability form offers dated slots and asks about adjustments. A job offer response form records the decision and the notice owed to a current employer, separate from the contract. A contractor onboarding form collects the ABN, GST status and insurance certificate, and a bank details form takes the BSB and account with a warning about payment redirection scams. An employee details change form lets staff update their address, name and emergency contact in one place.
Working
Five forms support the working relationship. A goal setting form limits each person to three measured goals. A one on one form records wins, blockers and an honest wellbeing check, with actions that roll forward. A flexible working request lists the grounds from the Fair Work Act and the 21 day written reply rule. A parental leave form separates government Parental Leave Pay from the employer's own paid leave and annual leave. A company car request records the purpose of each trip, which the tax treatment depends on.
Reviewing and leaving
Four forms cover the moments that need the most care. A probation review asks for evidence behind every rating and is used twice. A disciplinary meeting record follows the steps of a fair process, from the support person to the employee's response in their own words. A return to work plan works within the certificate of capacity and names who supervises each duty. A resignation form keeps the reason optional and explains final pay. Six of the fifteen forms are Australian and cite the Fair Work Ombudsman, Services Australia or WorkSafe Victoria. Most should be limited to signed in staff, since they hold personal information. Every business and person in the examples is invented.

- Company car request form for pool vehicles
A company car request form books a pool vehicle for business use: which car, the dates, why it is needed, the driver's licence and history, a fuel card and agreement to the vehicle policy. This version keeps private use off by default, because that is what the logbook and the tax treatment depend on.
- Contractor onboarding form for an Australian studio
A contractor onboarding form collects what a business needs before it can pay a contractor: the ABN and GST status, insurance, bank details, the services and the rate, and agreement to the contract terms. This Australian studio version will not pay an invoice until the form and the certificate of currency are on file.
- Disciplinary meeting record for a fair process
A disciplinary meeting record captures who attended, including any support person, the allegation as it was put, the evidence shown, the employee's response in their own words and the outcome. This Australian version is built around the steps that make a process fair, because those are what a tribunal looks at later.
- Employee bank details form with split pay
An employee bank details form collects the account name, BSB and account number that wages are paid into, the pay date the change starts from and the employee's signature. This Australian payroll version adds an optional fixed amount to a second account and warns staff about payment redirection scams.
- Employee details change form for address, name and contacts
An employee details change form lets staff tell HR what has changed in one place: their address, legal name, emergency contact or mobile, with evidence for a name change and the date it applies from. This version asks staff to tick what changed first, so they fill in only the sections they need.
- Employee goal setting form with three measured goals
An employee goal setting form records a small number of goals for a set period, how each will be measured, when it is due, how the goals connect to team priorities, what support is needed and when progress will be checked. This version limits each employee to three goals and asks both employee and manager to sign.
- Employee referral form with candidate consent
An employee referral form lets staff put forward someone they know for an open role: which role, the candidate's details, how they know each other, a CV, why the candidate fits and whether they have agreed. This version will not contact a candidate who has not agreed, which protects both the candidate and the referrer.
- Employee resignation form with notice and last day
An employee resignation form is a written resignation: the last day, how much notice is being given, a handover contact, the property to return and where to send final documents. This small cafe version keeps the reason for leaving optional and thanks the employee rather than asking them to justify leaving.
- Flexible working request form for Australian employees
A flexible working request form records which eligible ground applies, what change the employee wants, the proposed pattern, when it should start and for how long, and how the work will be covered. This Australian version lists the grounds from the Fair Work Act and states the 21 day written reply rule up front.
- Interview availability form candidates finish in a minute
An interview availability form asks a candidate which of a set of dated slots they could attend, how they would like to be interviewed, whether they need any adjustments and how to reach them. This conversational version does it in five screens, with a None of these work answer so nobody is forced into a time they cannot make.
- Job offer acceptance form with start date and notice
A job offer acceptance form records the candidate's decision on a specific offer, and if they accept, when they can start, how much notice they owe their current employer and anything they need to raise. This version keeps the decision separate from the employment contract, which follows for signature once the offer is accepted.
- One on one meeting form with rolling actions
A one on one meeting form records what a manager and a team member covered: recent wins, blockers, how the person is going, their development, actions with owners and dates, and when they meet next. This agency version is written by the team member, and its actions table rolls forward from meeting to meeting.
- Parental leave form for Australian employees
A parental leave form records the expected birth or placement date, the leave start and end dates, how the employee will be paid while away and how they would like to stay in touch and come back. This Australian version follows the Fair Work notice rules and separates government Parental Leave Pay from the employer's own paid leave.
- Probation review form with evidence for every rating
A probation review form records how a new employee is going against a small set of areas, the evidence behind each rating, the support they have been given and the decision: continue, confirm, extend or escalate. This version is used twice, at three months and before probation ends, so the end has no surprises.
- Return to work plan form for an injured worker
A return to work form sets out how an injured worker comes back: the certificate of capacity it follows, suitable duties and who supervises them, hours per week, restrictions and when the plan is reviewed. This Australian civil contractor version is signed by the worker and the supervisor, so both have agreed to the same plan.
