Employee resignation form with notice and last day

An employee resignation form is a written resignation: the last day, how much notice is being given, a handover contact, the property to return and where to send final documents. This small cafe version keeps the reason for leaving optional and thanks the employee rather than asking them to justify leaving.

What each field on this form collects

The last day and the weeks of notice share a row. An optional single choice asks the reason for leaving, with prefer not to say among the answers. A short text asks who should take over the employee's regular customers and recipes. A checklist covers keys, uniform, the staff discount card and the locker. A required address block says where to send the final payslip and tax summary, and the employee signs.

Notice is in the contract or award

In Australia, the notice an employee must give when resigning is usually set by their award, enterprise agreement or contract, not by the National Employment Standards, which set the notice an employer must give. The Fair Work Ombudsman's resignation page explains where to look and what happens to final pay. Because the right amount varies, the intro tells the employee where to find it and to ask before submitting. The form records what was given, which the owner can check against the award.

Leaving well

Small businesses rely on the knowledge of each person. The handover question asks who should take over regular customers and recipes, the kind of thing a cafe loses when someone leaves quietly. The property checklist avoids chasing keys after the last shift. Final pay, including unused annual leave, is paid out on termination, and the thank you screen says when. Keeping the reason optional is a choice: a business that wants to learn why people leave can ask in a separate exit interview, where the conversation is better than a tick box.

How to adapt this form

A larger business adds the manager's name, an exit interview invitation and IT equipment to return. A business with staff on visas adds a reminder that the employer may need to notify the immigration department. A company outside Australia replaces the award reference with its own notice rules. A business with seasonal staff adds a question about whether the person would like to be contacted for next season, which turns a resignation into a future roster.

What makes this form work

The reason is optional

Six reasons including prefer not to say, and none required. Nobody has to justify leaving to submit a resignation, which keeps the last weeks friendly.

A handover question that fits

Who should take over your regulars and recipes is a cafe question. It captures knowledge the business would otherwise lose with the person.

Final pay explained

The thank you screen says final pay, including unused annual leave, comes on the next pay day. The most common question after a resignation is answered before it is asked.

Questions people ask

How much notice does an employee need to give?

In Australia it is usually set by the award, enterprise agreement or employment contract. Many awards require the same notice from employees as employers must give, based on length of service. The Fair Work Ombudsman's resignation page explains how to check.

Does a resignation have to be in writing?

Not always legally, but writing it down avoids disputes about the last day and the notice given. This form is a written resignation with a signature and a time stamp, and the employee can keep a PDF copy of it.

What is included in final pay?

Outstanding wages, unused annual leave and in some cases long service leave, plus any other amounts owed under the award or contract. Unused personal leave is generally not paid out. Final pay timing may be set by the award or agreement.

Can an employer refuse a resignation?

An employee can resign by giving the required notice. An employer may agree to an earlier finish, or pay out the notice instead of having the employee work it. If an employee leaves without giving enough notice, some awards allow the employer to withhold an amount.

Should the employer ask why someone is leaving?

It can, and the answers help, but making it optional and asking in an exit interview usually gets more honest responses. This form keeps the reason optional and offers prefer not to say, so an employee who would rather not explain can still resign in a minute.

Can a resignation be withdrawn?

Only if the employer agrees. Once a clear resignation is given, the employer can accept it as final. If an employee changes their mind, they should talk to the employer straight away. Submitted responses cannot be edited, so the conversation is recorded separately.
