School uniform order form for a primary school uniform shop
A school uniform order form has to say which child, which class, which items in which sizes, and how the parcel and the money will meet. This one lists GST inclusive prices above a six row item table, sends orders to the classroom or the shop counter, and asks for the bank transfer reference so payment can be matched.
Self assessment form
A self assessment collapses when it is written the night before the meeting and asks for a rating first. This one asks for the record before the reflection, gives the misses their own question, and ends on the one thing a manager can act on immediately. It is a conversational form, asked one question per screen, because written answers get longer when nothing else is on the page competing for the box.
Shift swap request form
A shift swap is agreed in a corridor and then forgotten by everyone except the person who did not turn up. This request writes the whole arrangement down in eleven taps: the exact shift, the exact colleague, whether they actually said yes, and what is coming back the other way. It is conversational, because it is answered on a phone between tables, and because every answer is a tap or a single line.
Site inspection booking form for an electrician's quote
A site inspection booking form has to get a tradesperson to the right address, through the right door, with the right parts, at a time someone can let them in. This one asks the job type, access, a time window and parking, takes photos of the switchboard and the work area, and asks about pets before the gate opens.
Sleep consultant intake form with a three day sleep diary
No sleep plan can be written from one night, because one night is either the good one a parent remembers or the terrible one that made them book. This fifteen field intake form collects three days as a diary table, plus the room, the routine and the way the child actually falls asleep at bedtime.
Social media consent form for staff, platform by platform
A social media consent form for staff has to record where an employer may post photos of them, how they are named and how they can take that back. This one asks platform by platform in a five row grid, so a barista can say yes to the website and no to TikTok, and prints the withdrawal steps above the signature.
Software request form with a data and security check
A software request form lets IT check a new tool before anyone signs up: what it costs, how many people need it, what data it will hold and whether it works with company sign in. This one adds a required question about alternatives, so the request shows the team looked at what the company already pays for.
Spare parts order form with model, serial and part numbers
A spare parts order form has to identify the exact machine before the part, because the same model can take different parts across production runs. This one asks the model and serial number first, takes part numbers, descriptions and quantities in a numbered table, and accepts a photo when a part number is missing.
Speech pathology intake form as a child case history
A paediatric speech pathologist starts from a case history that reaches back to birth, because prematurity, early feeding and the age of first words each change what a delay is likely to be. This twenty one field intake form collects that history, the hearing testing, the languages spoken at home and the funding, before the first forty five minute session begins.

A roster is only as good as the availability it was built from, and most availability arrives as text messages that contradict each other. This form turns the whole question into one grid of days against shift blocks, then asks the four things a grid cannot express. It is a classic form, because a seven by four grid and a table of dates away need the width of a page rather than a phone screen.
Staff induction form
An induction record is only worth having if it says what somebody was actually shown. This checklist is written to be filled in while walking the site, with each safety line phrased as a place they stood rather than a document they were sent, and the licences and equipment recorded in tables with tags and expiry dates. It ends with both people signing, and it is written for an Australian worksite. Classic, because an inductor working from a tablet on the floor needs to see every section at once and come back to the ones not yet done.
Staff requisition form for a new role or a backfill
A staff requisition form asks approval to hire before a job ad goes out: which role, why it is needed, what it will cost and who signs off. This one separates a new position from a backfill, asks who is leaving when it is a backfill, and requires the position description as a PDF.

A university society lives or dies on its mailing list, and a mailing list full of expired student accounts is worse than a short one. This nine field membership form takes a student number, with a paragraph showing where it is printed on the campus card, then spends the rest of its questions working out which trips the committee should actually run.

A student satisfaction survey for a training provider has to show whether trainers, assessments and materials worked, and whether the course led anywhere for the learner. This template, written for an Australian registered training organisation, rates the trainer in one grid, asks if assessments were fair and ends with the employment outcome.
Supplier feedback survey: how your suppliers rate you as a customer
A supplier feedback survey asks the businesses that sell to you how you are to work with: whether you pay on time, forecast honestly and answer the phone. This one turns that into seven screens for a food manufacturer's procurement team, ending with whether each supplier wants to keep the account.
Supplier registration form built for a quality review
A supplier registration is reviewed twice, once by quality and once by procurement, and a form that only serves one of them stalls for weeks. This one carries a six line product table with real lead times, a certificate with its scheme and expiry, an allergen statement covering the whole site, and the commercial answers on the same page.
Surf lesson waiver on swimming ability and ocean conditions
A surf lesson waiver confirms that a surfer can swim, has declared anything that matters in the water and accepts the risks of the ocean before a lesson. This one asks an Australian surf school's students whether they can swim 50 metres unaided, then covers health, experience, conditions, wetsuit size and a guardian for under 18s.

A swim school takes a whole term in one go: the class time, the level the child swims at now, and the health facts a teacher needs while standing at the edge of the water. This form collects all of it in fourteen fields and finishes with a guardian signing the emergency treatment authority, so no term starts on a verbal yes.
T shirt order form with a size grid and print position
A t shirt order form has to collect how many of each size, which colour and where the print goes, because those three set the price and the screens. This one uses a six row size and quantity table, gives each print position its maximum width in centimetres, and takes the artwork as an upload with the file rules stated.
Tattoo booking request form that gives an artist enough to quote
An artist cannot quote a tattoo from a description of the idea alone, because price follows placement, size and skin rather than sentiment. This booking request asks for the longest side in centimetres, where on the body it goes and whether it sits over existing work, then takes the references as an upload.
Tattoo consent form signed on a tablet before the needle
A tattoo consent form is a health questionnaire, an age check and a design confirmation, signed in the same minute. This one does all three on a tablet, asks the eight health questions an artist needs, takes a photo of the ID, and files a PDF the studio can produce two years later.
Teaching job application form with registration and referees
A teaching application is checked against two registers before anybody reads the cover letter, which is why the numbers come before the prose. This eighteen field form asks for a teacher registration number and the state that issued it, a working with children check with its expiry, the subjects and year levels somebody will actually take, and three referees including a current line manager.
Team merch order form with name and number printing
A team merch order form has to collect the items and sizes for a bulk order, plus the name and number to print on each one, from every player before the order closes. This one does it for a netball club with member and non member prices, a printing section with the rules for names, and a plain note that the treasurer sends the total.
Team tryout registration form
Representative tryouts turn on two things a club usually finds out too late: what the player can safely do on the day, and whether the family has understood what a season actually costs. This registration asks both before anybody puts a bib on. It is a classic form, so a parent can see the health grid, the costs and both signatures on the one screen.
