Consent form templates, field by field
A consent form has to show that the person knew what they were agreeing to, agreed to each part separately where the parts differ, and signed it themselves. Each form here is finished and readable in full, with the reason every field is there.

Twenty three forms, four kinds of consent
Ten of the forms consent to a treatment or a health service: dental treatment, cosmetic treatment, laser hair removal, massage, counselling, telehealth, piercing, a fitness readiness questionnaire, a childcare medication authority and surgery on a pet. Four give permission to use a person's image or data: a childcare photo consent, a media release, a staff social media consent and a research participant consent. Seven are activity waivers for a gym, a trampoline park, a surf school, trail rides, a guided walking tour, an escape room and a go kart track. The last two give permission for a person or a place: a junior football club's season consent and a renter's consent to a trade visit. Treatment consents spend their fields on risks, alternatives and health questions, image consents on each separate use, and waivers on fitness, rules and who signs for a child.
Why every one is a classic form with a signature
All twenty three are classic forms, and each ends with a signature drawn or typed by the person filling it in, which prints into the response PDF. A conversational form can take a drawn signature too, but consent wording reads better on one page beside the place it is signed. A classic field's description is not shown on the published form, so the terms and the privacy note sit in paragraphs above the questions they cover. Where consent has parts, each part is its own question: the photo consent asks yes or no for four uses, the research consent ticks three data statements one by one, and the social media consent asks platform by platform. Four are always signed by a parent or guardian for a child, and five more add a guardian for a client under age.
Choosing one and adapting it
Start from the form whose risk is closest to yours rather than the one with the closest name, because a laser clinic and a massage room ask very different health questions. Nine of the forms are Australian and name the rules or guidance they follow; the rest are written for no particular country. No email goes to the person who signs, so if they need a copy, send it yourself from the response. A signed form records agreement. It does not replace the conversation about risks, and a term the law does not allow is not made valid by a signature. Every business and person in the examples is invented, and none of this is legal advice.

- Adventure tour waiver for fitness, medical and evacuation
An adventure tour waiver records that a guest understands the risks of a remote trip, has declared their fitness and health honestly and knows who pays if they need to be evacuated. This one does it for a guided multi day walking company in eleven fields, with a four level fitness rating and an evacuation cost acknowledgement.
- Cosmetic treatment consent form with an eight item risks checklist
A cosmetic treatment consent form records that a client understood the treatment, its risks and the alternatives before agreeing to it. This one, for a skin clinic offering peels and microneedling, lists eight risks to tick, then asks about alternatives, aftercare and before and after photos before the client signs.
- Dental treatment consent form with item numbers and fees
A dental treatment consent form has to record the procedure, what it will cost, the other options the patient heard, the risks they read and how they will be sedated. This one asks for each in turn, with a four row table for the item numbers and fees from the treatment plan and a signature at the end.
- Escape room waiver with a player table and a claustrophobia check
An escape room waiver records that the team has read the room rules and told the host about anything that could make a dark, enclosed room unsafe. This one is signed once by the lead player, with a six player table, a conditions checklist and the promise that doors are never locked.
- Go kart waiver with licence class, height and safety briefing
A go kart waiver records that a driver understands the risks of racing, meets the height rules for their kart and has been briefed on track safety. This one adds licence class, a medical checklist, footwear and a photo for the timing screens, with a guardian for drivers under 16.
- Gym waiver form with a health declaration, signed online at join
A gym waiver form records that a new member understands the risks of training, has declared any health conditions and accepts the club's terms before using the floor. This one is filled online at join for an Australian strength gym, with a six item health declaration, a clearance question and a signature.
- Horse riding waiver with rider weight, height and experience
A horse riding waiver records that a rider understands the inherent risks of riding and has given the details needed to match them with a suitable horse. This one asks a trail ride customer for weight, height and experience first, then a helmet choice, medical details, the risk acknowledgement and a signature.
- Informed consent counselling form, confidentiality first
Informed consent for counselling means the client knows, before the first session, what stays private, when it cannot, what it costs and how notes and contact are handled. This form puts the four limits of confidentiality first, each ticked separately, then fees, notes and contact, and ends with a signature and date.
- Laser hair removal consent form with skin type and sun exposure
A laser hair removal consent form records the facts that decide safe laser settings, such as skin type, recent sun and light sensitising medicines, and that the client understands the risks. This one asks twelve questions before the patch test, with all six Fitzpatrick skin types described in plain words.
- Massage consent form with draping, areas and the right to stop
A massage consent form records which treatment a client agrees to, which parts of the body may be massaged and that they can stop at any time. This one also explains draping in plain words, asks about pregnancy and health conditions, and takes the client's signature before the first appointment.
- Media release form for an interview, recording and attribution
A media release form is the written permission an organisation needs before it records someone and publishes their words or image. This one lets a person interviewed by a foodbank choose what is recorded, how they are named, where the story appears and whether they see it first.
- Medication consent form for childcare, dose by dose
A medication consent form for childcare has to authorise educators to give a named medicine at a set dose, time and route, and show when the child last had it. This one puts those four details in a table copied from the pharmacy label, then asks the reason, prescriber, expiry, storage and last dose before a guardian signs.
- PAR Q form online: seven questions, clearance and a signature
A PAR Q form is a short physical activity readiness questionnaire that asks seven yes or no questions to find people who should check with a doctor before exercising more. This one puts the seven questions online for a Pilates studio, then asks about clearance for any yes and takes a dated signature.
- Parental consent form for a whole club season
A parental consent form for a sports club has to say which activities the child may join, who may drive them, what happens if they are hurt and whether they can be photographed. This one covers a whole junior football season at once, with a medications table for the coach and a guardian signature.
- Photo consent form for a childcare centre, one use at a time
A photo consent form records where a child's photo may be used, for how long, and how a parent can withdraw that permission. This one asks Australian childcare families yes or no for four separate uses, whether a first name may appear, and whether there is a safety reason the child must not be identifiable.
- Piercing consent form with an ID check and aftercare
A piercing consent form records who is being pierced, where, with what jewellery, and that the client is old enough, has declared relevant health conditions and has received aftercare instructions. This one, for an Australian studio, adds a photo of photo ID and a guardian signature for clients aged 16 or 17.
- Property access consent form for a trade visit
A property access consent form records that a renter agreed to let a tradesperson in at a set time, with pets and keys sorted in advance. This one does that for a Victorian rental in eight fields, and its intro tells the renter what the notice rules would be if they did not agree.
- Research participant consent form with a withdrawal clause
A research participant consent form has to show that the participant read the information sheet, agreed to each use of their data separately and knew how to withdraw. This one does that for a university sleep study, with three data statements ticked one by one, a recording choice, a dated withdrawal cut off and both participant and researcher signatures.
- Social media consent form for staff, platform by platform
A social media consent form for staff has to record where an employer may post photos of them, how they are named and how they can take that back. This one asks platform by platform in a five row grid, so a barista can say yes to the website and no to TikTok, and prints the withdrawal steps above the signature.
- Surf lesson waiver on swimming ability and ocean conditions
A surf lesson waiver confirms that a surfer can swim, has declared anything that matters in the water and accepts the risks of the ocean before a lesson. This one asks an Australian surf school's students whether they can swim 50 metres unaided, then covers health, experience, conditions, wetsuit size and a guardian for under 18s.
- Telehealth consent form for video and phone sessions
A telehealth consent form has to record how the session will run, where the client will physically be, what privacy risks they accept and who can be called in an emergency. This one asks those eight things on one page for a psychology practice, with the crisis numbers printed above the emergency question.
- Trampoline park waiver with a jumper table and one guardian
A trampoline park waiver records that a parent or guardian accepts the risks of jumping for each child in the booking and has explained the park rules. This one lists up to four jumpers in a table, asks about the rules video, injuries and grip socks, and takes one guardian signature.
- Vet surgery consent form with bloods, fluids and estimate
A vet surgery consent form has to record what the owner agreed to: the procedure, the optional anaesthetic extras, the estimated cost and what to do if something goes wrong. This one asks each of those as its own question, including resuscitation, and keeps a contact number for the day above the owner signature.
