Form examples
Real forms, field by field
Every form here is finished and readable in full. The field lists come from how these forms are actually written in the trade, not from a template gallery, and each page says why a field is there before you copy it.

Printing order form with stock, finish and artwork
A printing order form has to pin down the product, the finished size, the paper, the sides, the finishes and the quantity, because a print shop cannot quote a job missing any one of them. This one asks all six in the order an estimator prices them, then takes the PDF, the proof choice and the date.

Private dining enquiry form with the minimum spend in writing
A private dining enquiry form has to settle the room, the menu and the drinks, because together they decide whether a group clears the minimum spend. This one prices each choice in its label and puts the minimum spend and deposit terms above a required tick, so the quote that comes back holds no surprises.

Product feedback survey template with a feature importance matrix
A product feedback survey has to find out which features customers rely on and what is missing, segmented by who they are and how long they have used the product. This template asks tenure, role and main use first, then puts six features into one importance grid, and closes with price fairness and a recommendation score.

Product order form
A wholesale order form template should capture the account, the order lines, where the goods go and where the invoice goes, and nothing else. This one is built the way a trade desk receives orders from existing trade accounts. It captures the business name and trade account number, who is ordering, a purchase order number, a five row order table with item code, product, variant and quantity, a delivery address with dock or level, a delivery method and requested date, and a billing address that is asked for rather than assumed. About four minutes.

Product registration form that captures the serial and the proof
A product registration is only worth collecting if it produces something a service desk can act on, which means a serial number in the right format and a receipt that is actually readable. This ten field form states the serial format, tells people where the compliance plate is, and separates a recall notice from a marketing email.

Product sample request form with project details
A product sample request form has to say which samples to send and where, and give the supplier enough about the project to know which requests are a future order. This one lets a visitor pick up to five tiles, then asks the project type, its stage, the area in square metres and whether they are in the trade.

Property access consent form for a trade visit
A property access consent form records that a renter agreed to let a tradesperson in at a set time, with pets and keys sorted in advance. This one does that for a Victorian rental in eight fields, and its intro tells the renter what the notice rules would be if they did not agree.

Psychology intake form with the limits of confidentiality on it
A first psychology session has fifty minutes and a person who has usually waited weeks for it, so the history should not eat the first twenty. This twenty field intake form takes the referral pathway, a ten item distress baseline and the supports already in place, and states the limits of confidentiality above the signature.

Purchase requisition form with line items and an approver
A purchase requisition form asks permission to buy before anyone orders: what is needed, what it costs, which budget pays and who signs off. This one lists the items in a four line table with unit cost and supplier, takes the total and budget code, and lets the requester pick the approver by spend limit.

Raffle ticket form
A raffle is a cash transaction that has to survive a draw four weeks later, so the form has one real job: produce a record both sides will still trust when a number is read out in a hall. This one collects the name that gets called, the address the numbers go to, and the seller who should be credited. It is conversational, because most tickets are bought standing at a market stall with a phone in one hand. Raffle rules in Australia are set by each state; this example is a Victorian brigade, where only a declared community or charitable organisation may run a raffle and buyer contact details have to be recorded.

Reference check form
A reference done by phone produces notes nobody can find and a memory of a tone of voice. Sent as a form, it produces the same four facts every time, a rating grid the referee can honestly leave blank, and the rehire answer in the referee's own words. This one is written for UK employers, and it is a classic form, because a referee wants to see the whole thing before starting and answer the parts they can.

Reimbursement request form
A community club runs on people who buy things with their own money and then feel awkward asking for it back. This request makes asking straightforward and gives the treasurer the four things a set of club books needs: the budget line, the approver, the receipt and a payment method. It is a classic form, because the treasurer's rules sit in paragraphs right above the fields they govern, and the receipt is the whole point.

Remote work survey template: setup, focus and connection
A remote work survey has to show how people actually split their week and what each place is good for, before a company sets or changes its hybrid policy. This template asks days remote and commute, checks the home setup, compares focus at home and in the office, and asks what the office is for.

Rental application form
A rental application form template collects the property and offer, the applicant's identity, their current and previous address, employment and income, references and signed consent to screening. This one is a residential application, one per adult, that takes about ten minutes with identification and payslips to hand. It covers occupants and pets, agent contacts, an employment table, a proof of income upload, two non family references, and a signed consent to a tenancy database and credit check.

Reorder form for repeat customers with account and items
A reorder form has to let a customer who already knows what they buy send the same order again fast, using their account and the product codes on their last invoice. This one fits on one screen: account number, who ordered, six lines of code and quantity, delivery, a date, the purchase order and notes.

Repair request form with photos and warranty check
A repair request form gives a service team what it needs to book a repair: which product, its serial number, what is wrong and whether the warranty covers it. This one adds optional photos of the fault, a proof of purchase upload for warranty claims, and a choice between dropping the appliance off and a courier pickup.

Request a quote form
Most quote request forms collect a name, a phone number and the sentence "I need a fence", which guarantees a call, then a visit, then a price. This one asks for the four things that actually let a trade put a number on a job: a measured run, a photo down the whole line, the ground conditions and a budget band. It is a classic form, because the photo field and the measurement have to be visible together while somebody stands at the job.

Research funding application form with a justified budget table
An internal seed fund is judged by a cross disciplinary panel that reads the lay summary aloud and argues about the budget, so those two parts of the form need the most structure. This fifteen field application asks for a 300 word lay summary, six budget lines each with a written justification, and an ethics status that includes not sure yet.

Research participant consent form with a withdrawal clause
A research participant consent form has to show that the participant read the information sheet, agreed to each use of their data separately and knew how to withdraw. This one does that for a university sleep study, with three data statements ticked one by one, a recording choice, a dated withdrawal cut off and both participant and researcher signatures.

Restaurant feedback form with a course by course rating grid
A restaurant feedback form has to tell the kitchen and the floor team which part of a visit worked and which did not, without making diners write an essay. This one puts every course plus service, ambience and value into one rating grid, then asks for the server, the best dish and whether they would return.

Restaurant reservation form with the occasion and the seating
A restaurant reservation form has to settle the night, the time, the number of covers and where the party sits, because those four decide whether the table exists. This one adds the occasion, the dietary needs and a high chair, so the floor manager can place the booking before service instead of during it.

Routine property inspection form done room by room on a phone
A routine inspection is a walk through five rooms with a phone, and the report is only worth anything if it is written during the walk, not from memory in the car. This form is the walk: one condition grid per room, one photo per area, a maintenance list, and a PDF at the end for the owner and the tenant.

Salon booking form that asks what has been on your hair
A salon booking goes wrong when the slot booked and the service required are different lengths, and colour is where that gap gets expensive. This booking form puts the real duration beside every service, asks what has been on the hair in the last twelve months, and checks whether a patch test exists before anybody is offered a colour appointment.

Scholarship application form that asks everyone the same three questions
A scholarship committee reads a hundred applications against one rubric, and the only fair form is the one that asks everyone the same questions at the same length. This form checks eligibility first, then asks three questions with 250 word limits, takes the evidence as uploads, and ends with a declaration two people sign.
