Private dining enquiry form with the minimum spend in writing
A private dining enquiry form has to settle the room, the menu and the drinks, because together they decide whether a group clears the minimum spend. This one prices each choice in its label and puts the minimum spend and deposit terms above a required tick, so the quote that comes back holds no surprises.

What each field on this form collects
Your name, mobile and email come first, the email labelled as the one the quote goes to. The date and the arrival time sit on one row. Number of guests is a number field. Which room is a radio of three: the Barrel Room for 8 to 14 with a $1,400 minimum, the Vault for 15 to 30 at $3,000, and the Vault on a weekend night at $4,500. Set menu is a radio of three priced per head. Beverage package is a radio of four, from a bar tab on consumption to matched wines. Dietary needs is a long text that asks for numbers, not just names of diets. Audio and visual is an optional checkbox. A paragraph sets out the minimum spend and the deposit, and a required tick confirms it was read.
How the minimum spend works on this form
The minimum spend is the floor for the room, food and drinks together. Eighteen guests on the $95 menu with the house package spend $2,790, which clears the Vault minimum on a weekday and falls short on a Saturday, where the gap of $1,710 would be charged as a room fee. Putting the prices in the option labels lets the person enquiring do that sum before sending, and the paragraph above the tick explains what happens when the bill comes in under. The form does not total anything itself; the events manager confirms the figures in the quote.
How to adapt this form
A hotel with a function room adds an accommodation question and a cost centre for corporate bookings. A restaurant without a set menu requirement replaces the menu radio with a long text for the menu conversation and keeps the minimum spend tick, which is the line that protects the room. A venue that hosts wakes or memorials changes the welcome screen tone, shortens the notice period in the paragraph and drops the AV options to a single microphone question.
What makes this form work
Every choice carries its price
Room options state their guest range and minimum spend, menus state a price per head, and drinks packages state a price per head for three hours. A person can work out whether eighteen guests clear $3,000 before pressing send, instead of discovering it in the quote.
Weekend pricing is its own option
The Vault appears twice, once at $3,000 and once on a Friday or Saturday at $4,500. The higher weekend floor is visible at the point of choosing, which ends the back and forth that starts when a quote arrives higher than the website suggested.
The rule is read before the tick
A paragraph explains the minimum spend, the room fee, the 25 percent deposit and the 14 day refund line, and a required checkbox sits directly under it. The enquiry cannot arrive without that acknowledgement, so the conversation with the events manager starts from agreed terms.
Questions people ask
What is a minimum spend in private dining?
It is the least the room will cost, food and drinks combined, for the night. If the group spends less, the difference is charged as a room fee. It exists because a private room taken by eight people on a Saturday could have held thirty diners in the main restaurant, and the minimum spend covers that.
Why does the enquiry ask for numbers with each dietary need?
Because the kitchen orders and preps by portion. Two vegetarian and one coeliac guest is an order; vegetarian and coeliac is a guess. The placeholder shows the expected format, and the events manager reads it before quoting, since a set menu with six dietary variations can change the price per head.
Is the deposit taken through the form?
No. A form collects answers and does not take payments. The thank-you screen says the reply holds the room for five days with a written quote, and the booking is confirmed when the deposit is paid. The paragraph above the acknowledgement tick states the deposit as 25 percent of the minimum spend, refundable more than 14 days out.
Why is the minimum spend acknowledgement required?
Because the most common dispute in private dining is a group that spent under the minimum and did not expect the room fee. A required tick under a plain paragraph means every enquiry that arrives has read the rule. It is not a contract on its own; the written quote and the deposit confirm the booking.
Can the guest count change after the enquiry?
Yes, until seven days before the dinner, which the thank-you screen states. After that the kitchen has ordered, so the final number is what is charged. The number field on the form is the starting point for the quote and for choosing the room, not the final count.
Why does the form ask for an arrival time as well as the date?
Because the kitchen paces a set menu from the moment guests sit down, and a private room is often turned for a second group or closed early. The arrival time tells the events manager when canapes or the first course go out, and whether the booking fits around a lunch in the same room.

DJ booking form with set times and a must play list
A DJ booking form has to settle the date, the venue and exactly when the music starts and stops, then what the night should sound like. This one adds a five row must play table of songs and artists, a do not play list, and a checklist of the equipment the venue already has, so the DJ packs the right van.
Photo booth booking form with the print layout and props
A photo booth booking form has to pin down the date, the hours and the venue, then the three choices that change what guests take home: the booth, the print and the props. This one describes each booth and print in its label, states the floor space and power needed, and takes a logo for branded prints.
Tour booking form that asks about fitness before the date
A day tour is ruined for everybody when one person is on a walk that is two grades harder than they expected, and no booking system catches that. This tour booking form names the distance, the climb and the grading in every option, then asks the group to describe the least fit person coming, in words that make the honest answer easy to give.
