Classic form
Request a quote form
Most quote request forms collect a name, a phone number and the sentence "I need a fence", which guarantees a call, then a visit, then a price. This one asks for the four things that actually let a trade put a number on a job: a measured run, a photo down the whole line, the ground conditions and a budget band. It is a classic form, because the photo field and the measurement have to be visible together while somebody stands at the job.

What makes this form work
It asks for a measurement, and says how accurate is accurate enough
Metres, to the nearest five, paced out if there is no tape. Telling somebody the tolerance is what gets the number filled in. A blank asking for an exact length gets skipped, and a skipped length is a site visit.
One photo is specified rather than requested
Stand at one end, shoot down the line, get the ground in frame. The photo field description is one of the few pieces of help text a classic form actually shows a respondent, and using it to direct a single shot produces something that can be priced from.
The ground checklist covers what changes the price
Vehicle access, a step in the run, rock or fill, services in the line, a pool inside the boundary, a dog on site. Each one is a cost or a delay a quote has to account for, and each is a tick rather than a question somebody has to think how to phrase.
Questions people ask
Will people really answer a budget question?
More than you expect, if you say what it is for. The paragraph above the field explains that the band decides which material gets quoted, not how hard the price gets pushed, and the last option is an honest "I have no idea, that is why I am asking". A band with an escape hatch gets answered; a blank field asking for a number does not.
How many photos should a quote form ask for?
One that is compulsory and specific, plus a place for the rest. The single photo field here asks for one shot down the whole run and explains exactly how to take it, and the separate upload takes up to six more. Asking for six photos up front reads as work; asking for one with instructions gets a usable picture.
Does the form work on a phone while standing at the job?
That is what it is designed for, and it is why the measurement, the photo and the ground questions are grouped in that order. On a narrow screen the paired fields stack into one column, so the sequence a person meets is the same as on a desktop, just taller.
What happens to the photos in the response?
They are attached to the response and shown in the responses view, and the response PDF renders uploaded images inline and merges in any PDF that was attached. So the quote can be prepared from one document rather than from a form on one screen and an email of photos on another.

Make my request a quote formOther form examples
Demo request form
A demo request form that opens with company size and budget is asking the buyer to qualify themselves before they have any reason to. This one takes the three contact fields first, then spends five questions working out who should run the session and what to actually show. It is conversational, one question per screen, because a booking form on a phone is finished when nothing on screen looks like work.
Lead capture form
A lead capture form has one job: find out quickly whether there is anything here for either side. This one asks nine questions, three of which are the contact details and five of which are genuine qualifiers, and it tells people outright what spend level makes the numbers work rather than booking a visit to break the news. It is conversational, because a short qualifying form that shows one question at a time gets finished on a phone.
Contact form template
A contact form gets worse with every field added to it. This one has six, and the two doing the real work are the subject dropdown that routes the message and the line printing the reply time where somebody can see it before they write. It states the phone number rather than hiding behind the form, and it does not sign anyone up to anything. Classic, because six fields on one screen is faster than six screens.
