Invitation order form with wording, quantity and proofs
An invitation order form has to collect the exact wording, spelled as it will print, plus the count, the envelopes and how many proofs it takes to get it right. This one separates the names and the date line from the full wording, explains household counts, and takes the guest list as a PDF for addressing.

What each field on this form collects
Invitation suite is a dropdown of five designs, beside a quantity from 25 to 400. A paragraph advises one invitation per household plus ten spares. The names as they should appear and the date, time and venue line sit side by side as short texts. The full wording is a required long text with a placeholder that starts a traditional line. Envelope colour is a radio of six in three columns. A paragraph explains exporting the guest list to PDF, then the guest list upload takes one optional PDF. Proof rounds is a radio of three, beside the needed by date. An email for proofs closes the form. Nine of the ten fields are required.
Why the names and date line are asked twice
They appear once on their own and again inside the full wording, and that is on purpose. The two short fields give the studio the spelling and the date to check the wording against: if the long text says Saturday and the date line says Friday 11 December, the proof catches it before print. Names are the most common reprint, often a missing accent or a nickname used by mistake. The thank-you screen asks for a second reader on the proof for the same reason. Two rounds are included because most couples change one thing after seeing the first layout.
How to adapt this form
A save the date order drops the full wording and proof rounds and adds a magnet or card choice. A studio selling day of stationery adds menus, place cards and a seating chart, each with a count. A business sending corporate event invitations swaps names for the host company and adds RSVP details and a dress code. A studio printing in two languages adds a screen for the second language wording, proofed separately by a native speaker.
What makes this form work
Names and date are checked twice
Short fields for the names and the date, time and venue sit above the full wording. The studio compares them line by line, and mismatches surface at the first proof.
The guest list rule is honest
A paragraph says the upload takes PDF only and explains how to export the spreadsheet. Couples are not left wondering why their file will not attach.
Counts are by household
The paragraph under the quantity says one per household plus ten spares. The most common ordering mistake, counting guests, is answered before the number is typed.
Questions people ask
How many wedding invitations should I order?
One per household rather than per guest, so a couple and their children receive one invitation. Add about ten spares for keepsakes, the photographer and late additions. The paragraph on the form says this because a reprint of ten later costs nearly as much as the first run.
Can I upload my guest list as a spreadsheet?
Not directly. The upload on this form accepts PDF only, because the file field allows images and PDF and nothing else. The paragraph above it says to export the spreadsheet as a PDF with one household per line. The studio reads names and addresses from that PDF to set the envelopes.
How many proof rounds do invitations need?
Usually two: one to see the layout and one to confirm the changes. Couples with complex wording, two languages or many family names often need more, which is why four rounds is an option. A printed sample shows the card weight and colour, which a screen proof cannot.
When should wedding invitations be posted?
Commonly six to eight weeks before the wedding, and earlier for a destination wedding or a date in a busy season. Printing takes ten business days after final approval, so the needed by date should allow for proofs too. The thank-you screen states both timings so couples can work backwards.
Do square invitation envelopes cost more to post?
Often, yes. Postal services price letters by shape and size, and square or rigid envelopes are usually treated as non standard because they cannot go through sorting machines. Couples choosing a square suite should check the postage with their postal service before buying stamps for the whole list.
What if a name is wrong after printing?
The run is reprinted, which is why printing waits for a final approved proof. The two short fields for names and the date line exist so the studio can check the wording against them. Responses to the form cannot be edited after submit, so a correction goes by email to the studio before approval.

Balloon order form with colours, garland length and setup
A balloon order form has to capture the product, the colours, the size in metres and whether the decorator installs on site, because those four set the balloon count and the time on the day. This one gives a length guide by wall and table, limits colours to four in the label, and asks for a photo of the space.
Bakery pre order form for pickup day and time
A bakery pre order form has to tell the bakers how many of each loaf and pastry to bake for which morning, and tell the counter whose bag goes on the shelf and when they will come. This one starts with an item and quantity table, asks the pickup day and an hourly window, and settles slicing and packing before the dough is shaped.
Engraving order form with a text proof and font choice
An engraving order form has to get the exact words, spelled and spaced as they should be cut, because an engraved mistake cannot be fixed. This one limits each of two lines to 30 characters, asks the font and layout as named styles, and sends a proof for approval before any item is engraved.
