The USI is the field the whole booking turns on
Nationally recognised training in Australia is issued against a Unique Student Identifier, and a student without one has sat the course for nothing until they create it. So it sits high on this form, with a ten character pattern on the field and a paragraph underneath saying it is free, takes a few minutes and lives at usi.gov.au. The name field above it carries the same weight for the same reason: the statement of attainment is issued in that name, and a certificate in the wrong name is a reissue request rather than an afternoon of paperwork.
Asking for support before somebody has to ask out loud
First aid is assessed practically. Two minutes of uninterrupted compressions on a manikin on the floor, plus a written component, is a real barrier for somebody with a bad knee, limited English or a hearing aid, and almost none of them will raise it in front of a room. The rating grid turns four separate confidences into one compact question, and the adjustments box underneath takes the physical side. The paragraph above both says exactly what the assessment involves, which is what makes the questions answerable rather than vague.
How to adapt this form
A trainer delivering on client sites replaces the session dropdown with a requested date and a site address, and adds a headcount, since the booking is for a group rather than a person. A refresher only offering keeps the expiry date field and makes it required, because that is the whole eligibility test. An organisation booking its own staff drops the payment block entirely and adds an employee number and a cost centre, which is what the training coordinator actually needs back in the export.

The pattern stops pasted spaces and dashes, and the paragraph underneath says it is free and where to get one. A provider that asks for a USI without explaining it collects blanks from exactly the first time students who most need the prompt.

How many fields does a course booking need?
Fourteen here and eleven required, which is more than a simple booking because a registered training organisation has reporting duties a yoga studio does not. The optional three, the current certificate expiry, the employer billing line and the middle name, each exist for one group of students rather than all of them.
What is a Unique Student Identifier and why is it required?
It is a ten character reference that every student in nationally recognised Australian training holds, and it is what the certificate is issued against. It is free, it takes a few minutes at usi.gov.au, and a provider legally cannot issue the statement of attainment without it, so the form asks for it rather than hoping.
Can this form take the course fee?
No. A third of these bookings are employer invoiced or funded, and taking a card from a student whose workplace is paying creates a refund on day one. The form records the booking, the pre course work is sent within a day, and the invoice follows to whoever the payment question named.

What does the pattern on the identifier field actually do?
It holds the field to ten letters and numbers, which stops the most common error, a space or a dash pasted in from an email. It cannot tell whether the identifier is real. The provider still validates it against the registry before the session, and the acknowledgements say identification is checked on the day.
Does attending the course mean passing it?
No, and the third acknowledgement says so in plain words. Competency is assessed on the day, including the two minutes of compressions. Saying that at booking rather than at the end of a long Saturday is the difference between a student who prepared and a student who feels ambushed.
