Gym member survey questions that spot who is about to leave
Ask 10 to 12, and send it once members have trained for at least a month. Never skip how often they visit, how likely they are to still be a member in three months, and one open question about the change that would bring them in more. Those three find the members worth a phone call.
A gym member survey has to find out who is quietly drifting away before they cancel, and whether the floor, the timetable or the staff are the reason. These questions tie every rating to how often the member actually trains, so a complaint about crowding can be read against when and how often they come.

Which questions to make required
Only two questions are required. Visit frequency is the lens every other answer is read through, and the three month question is the one a retention call list is built from, so a response without either cannot be sorted. Equipment, class and staff ratings stay optional, because a member who only lifts weights should not have to rate a spin class to reach the end.

Three ways to change this bank
For a 30 day check in with new members, replace the equipment section with induction questions: was a program explained to you, do you know how to book a class, and what is the first goal you want to reach. Keep the three month question, because early drop off is where it earns its place. For a cancellation survey sent only to members who have left, cut to five: the main reason, how often they trained in their last month, what would have kept them, whether a pause would have suited them better, and whether they would come back at a lower price. For a pool or aquatic centre, turn the area grid into lanes, water temperature, change rooms and swim school, and ask when the lanes feel most crowded.
Questions people ask
When should a gym send a member survey?
Once a member has trained for at least a month, and then every six months. Earlier than a month, members are rating the sign up and the induction rather than the gym. Avoid January, when the floor is at its busiest and every rating about crowding is lower than the rest of the year would suggest.
Should a gym member survey be anonymous?
Make identity optional and ask for it last. Members are blunter about staff and cleanliness when nothing has asked their name, and the ones who want a reply can still leave it. Ask permission to contact before the name box, so a member who says no is never asked for a name at all.
What does the three month question show that a recommendation score does not?
It asks about the member's own plans, not their opinion of the gym. Plenty of members would recommend the place and still expect to cancel because their shifts changed or they moved. The three month answer, read next to visit frequency, gives a list of people to call this week, which a recommendation score cannot.
How do the ratings come out when exported?
The response export is a CSV with one column per question. Choice questions export the option labels, and the area grid arrives as a single column with each row written as the area and its answer, separated by pipes. Split that column before averaging, and filter out did not use first.
Should the survey name individual trainers?
Not in a general member survey. Rating named staff in front of every member invites personal comments and puts people off answering honestly. Ask about the team as a whole here, and if personal training matters, send a separate short survey to each trainer's own clients after a block of sessions.
How many responses are enough to act on?
Enough in each group you plan to compare. If off peak members and full access members are to be read separately, each group needs a few dozen answers before a difference means much. Below that, read the open answers as comments rather than as a trend, and look for the same complaint repeated.
