Twenty questions is the full bank and ten is what a short workshop should actually send. Three are essential: whether the workshop matched what the listing described, what the single most useful thing learned was, and whether the attendee would come to another one. The first protects your refunds, the second protects your reputation and the third fills the next calendar.
A two hour workshop earns a two minute survey, and anything longer will be abandoned by the people whose opinion you most wanted. These questions cover whether the session matched its listing, how it was paced, what was useful and what to run next.

Which questions to make required
Four things are required and they are all one tap or one line. The overall rating and the matched the listing question are required because together they separate a bad workshop from a mis sold one. The most useful thing learned is required because it is the only question whose answers can be quoted in next term listing. Would you come to another workshop is required because it is the headline. Everything else, including the whole parts of it grid, stays optional so the survey still finishes in two minutes.

How to change this for a different kind of session
A multi day course. A survey at the end of day three cannot remember day one, so send the same four questions each evening and a longer one at the end. Add a question about whether each day built on the last, and ask on the final day which day should be cut or extended, which is the only way to reshape a multi day programme without guessing. A free community session. Price and refunds are irrelevant, so replace them with questions about whether the session was easy to find, whether the venue was accessible, and whether the person would have paid for it. The last one is uncomfortable and it is the question a funding application will eventually ask you. A workshop for a corporate client. The attendee and the buyer are different people and want different things. Keep this survey for attendees, then ask the client three questions of their own: whether the session addressed the problem they described, whether they would book it for another team, and what they will do internally to follow it up. Never merge the two, because a room full of happy attendees and a client who saw no change is a specific and common failure.
Questions people ask
How soon after a workshop should feedback be asked for?
The same evening or the next morning, while the session is still vivid. A week later the answers describe whether the technique worked at home, which is a genuinely useful and completely different survey. Send both if you can, but never combine them into one long form.
Does the matched the listing question really matter?
More than the rating does. A good session that was not the advertised session produces refund requests and public reviews, and a satisfaction score cannot separate the two. It is also the single cheapest thing to fix, because the answer is usually a paragraph of listing copy.
Should a small business ask for a review in the survey?
Ask whether they would come again first, then invite only the yes answers to review elsewhere. Putting a review request inside the feedback form suppresses exactly the criticism the survey exists to collect, and attendees notice when a feedback form is really a marketing funnel.
How many screens is too many?
Past ten for a two hour workshop. The instrument here is eight screens because two of them carry a pair of related questions, which is the trick that keeps the count down without losing anything. Mostly one tap answers matter more than the exact number of screens.
Is a confidence question worth asking?
Yes, as the nearest thing to a learning measure a short session can carry. Asking how confident somebody feels doing it alone now is more honest than asking whether they learned a lot, and the answer tells you whether the hands on time was sufficient or merely present.
What response rate should you expect?
Forty to sixty per cent for a small paid workshop where the attendee met the teacher, which is far higher than anything a large organisation sees. Personal contact is doing that work, so send the survey from a person and not from a booking system.
What should the next topic question offer?
Real candidate topics you are prepared to run, not an open box. A blank field collects requests for things that would need equipment you do not own. A checklist of five plausible sessions turns the feedback form into the demand research for next term calendar.

A graduate survey has one job, which is finding out what people are doing now and whether the qualification had anything to do with it. Everything else, the salary question included, is optional and should be written as though it is.
