How to add a signature to a Google Form

Google Forms has no signature question type, so you cannot add a drawn signature field directly. The usual options are a typed name with a consent checkbox, a file upload where people attach a photo of their signature, a third party add-on, or a different form builder that includes a signature field. Each gives different evidence of who signed.

The question is common because Google Forms is free and familiar, and the answer is a workaround. Which workaround fits depends on whether you need a record of agreement or a signed document.

  1. 1

    Decide what evidence you need

For a simple acknowledgement, a typed name and consent box is usually enough. For a document the law or your insurer expects to be signed, use a proper signature field or an e-signing service.

  2. 2

For a typed signature, add two questions

Add a short answer question asking for the full name as a signature, and a required checkbox question stating exactly what the person agrees to.

  3. 3

For an image, add a file upload question

Add a file upload question limited to images and tell people to photograph their signature; note that they must sign in to a Google Account to answer it.

  4. 4

    Or install an add-on

Search the Google Workspace Marketplace for a signature add-on, check what data it accesses, and test it on a copy of the form.

  5. 5

Or rebuild the form with a signature field

Use a form builder that offers a signature component, so the signature is drawn in the form and stored with the response.

Why there is no signature field

Google's help page lists the question types Google Forms supports: short answer, paragraph, multiple choice, checkboxes, dropdown, file upload, linear scale, rating, multiple choice grid, checkbox grid, date and time. None of them is a drawing pad, so a drawn signature has to come from somewhere else. That is a design choice rather than a missing setting, and no option in the form editor turns one on.

The typed name route

Electronic signature laws, such as the US ESIGN Act and Australia's Electronic Transactions Act 1999, generally accept a signature in electronic form when it identifies the person and shows their intention, by a method that is reliable for the purpose. A typed full name next to a clearly worded consent statement, collected with the response time, can meet that for many low risk agreements. It is weaker evidence than a signature tied to a verified email and an audit trail, so use it for acknowledgements rather than contracts of real value.

The file upload route

A file upload question lets respondents attach a photo of their signature. Google's help page notes that responders must sign in to a Google Account to answer it, and uploaded files are stored in a new folder in the form owner's Drive. That links the upload to an account, but the image could be any signature photo, and many respondents without a Google Account cannot complete the form at all.

Common mistakes

Asking people to type their signature without saying what they are agreeing to. Making a signature image the only proof for a document such as a medical consent or a lease. Installing an add-on without checking what it can read. And forgetting that a form response is not a signed document: if the other party needs a signed PDF, a form alone does not produce one.

Signatures in this product's forms

The OneCraft form builder has an inline signature field, drawn or typed by the person filling the form and printed into the response PDF, which also works in conversational forms. Classic forms can also carry an e-signature field: with the e-signing add-on, a submitted response becomes a signing envelope with the signer's email from a fixed address, another field or asked at fill time, plus signing order and a certificate of completion.

Questions people ask

Is a typed name on a Google Form a legal signature?

It can be, for many everyday agreements, when the form makes clear the typed name is a signature and states what the person agrees to. Courts look at identity and intent. For contracts of real value, consents in health care or anything a regulator prescribes, a stronger method with an audit trail is safer.

Can respondents draw a signature on a phone in Google Forms?

Not in Google Forms itself, because no question type provides a drawing pad. Some add-ons offer one, and other form builders include a signature field that works with a finger on a phone screen. A file upload of a photo is the only built in way to capture an image.

Why do people need a Google Account to upload a signature image?

Because Google Forms requires responders to sign in before answering a file upload question, so the files can be stored in the owner's Drive and tied to an account. This blocks anonymous uploads, but it also stops anyone without a Google Account from finishing the form.

Can I turn Google Form responses into signed PDFs?

Not directly. Some add-ons generate documents from responses, and some send them to an e-signature service. Alternatively, collect the details in the form and send the finished document for e-signature separately. Either way the signature happens on the document, not inside Google Forms.

What should a consent statement next to a typed signature say?

Exactly what the person agrees to, in plain words, with any document they are accepting linked or quoted, followed by a required checkbox. For example: I have read the excursion details above and give permission for my child to attend. Typing my full name below acts as my signature.
