This agreement is made on 5 October 2026 between Sentinel Pest Management Pty Ltd, ABN 77 204 663 118, of 5 Argyle Place, Marrickville NSW 2204, called the Technician, and Bellota Hospitality Pty Ltd, ABN 19 662 330 845, trading as Bellota Restaurant at 212 Illawarra Road, Marrickville NSW 2204, called the Client.

1. The premises and the programme
1.1
What is covered
The whole of the tenancy at 212 Illawarra Road, being the kitchen, cool room, dry store, bar, dining room, staff area, both toilets and the rear bin yard, about 210 square metres. The programme is preventive: four scheduled visits a year plus call outs under clause 1.3.

Scheduled visits
Visits fall in the third week of January, April, July and October, on a Monday before 9 am while the kitchen is closed. Each visit is an inspection of every zone, a treatment to the table in clause 2, a check and rebait of the 14 numbered stations, and a written report left with the manager before the Technician leaves.

Call outs between visits
A call out for a live sighting is attended within one business day and is included in the fee, up to three call outs a year. A fourth and any later call out in the same year is charged at $145 for the visit plus product at cost, quoted before it is done.

Safety data sheets
The Technician leaves a current Safety Data Sheet for every product used in the site folder and emails a copy to the manager after the first application of any new product. The folder also holds the label directions, so a food safety auditor can see what was applied, where, at what rate and on what date.
Re-entry times are not a suggestion
The kitchen does not reopen before the re-entry period in the table has passed, measured from the end of the application, not the start of the visit. The Technician writes the clearance time on the report and tells the manager before leaving.
3. What each side does
3.1
The Client
Gives access before 9 am on the scheduled day, covers or removes open food, empties the bin yard the night before, keeps the 14 stations clear of stock, reports a sighting the day it happens, and does not move, open or discard a station or a monitor.
3.2
The Technician
Holds a current pest management technician licence and shows it on request, carries public liability cover of $20 million and workers compensation as required by law, leaves a written report every visit, and reports any structural or hygiene issue that is feeding the problem rather than treating around it.
3.3
Records for an audit
The Technician keeps a service history for this site for seven years and supplies a summary within two business days of a request. The site folder holds the last 12 months of reports, the station map and the Safety Data Sheets, which is what a food safety audit asks to see.
4. Fees

The quarterly fee
The fee is $180 plus GST a quarter, invoiced after each visit and payable within 14 days. It covers the visit, the products in the table, the station service, the report and the first three call outs a year.

Price review and extra work
The fee may rise once a year by no more than the annual change in the Sydney Consumer Price Index, on 30 days written notice. A rodent proofing job, a fumigation or a heat treatment is quoted separately and is not part of this agreement.
5. Standard of work, term and general
5.1
What is promised, and what is not
The services carry the consumer guarantees under the Australian Consumer Law, including due care and skill. A preventive programme reduces pressure and finds problems early; it does not guarantee that no insect ever enters a restaurant with a delivery. Where activity continues after a treatment, the Technician returns at no charge and reviews the programme rather than repeating the same application.
5.2
Term, ending and whole agreement
This agreement starts on 19 October 2026, runs for 12 months and continues until either party ends it on 30 days written notice, with any visit already scheduled still carried out. A party in serious breach has 10 business days to fix it after notice. This agreement is governed by the law of New South Wales and replaces the quotation dated 18 September 2026.

How to adapt this agreement

For a warehouse or food manufacturing site, replace the restaurant zones with docks, racking aisles and the perimeter, add a bait station map as an annexure, and raise the reporting to monthly with trend data by station. For a residential termite programme, swap the treatment table for an inspection schedule under the relevant Australian Standard, add a clause on what an inspection cannot see, and replace the quarterly fee with an annual inspection fee plus a treatment quote.
