Write the form for the captain, not for the crowd
Sixty people come to a school trivia night and about eight fill in a form, because one person books each table. That changes what the questions should be. The captain needs to know the table price in seats rather than in total, needs to be told what happens to empty chairs, and needs somewhere to say that two friends want to come but not sit with them. Name, email and mobile belong to the same person and are filled from memory, so a profile block takes them together. The description says the captain is who gets contacted, which quietly stops a parent putting down somebody who is not coming.
