Form questions
Member satisfaction survey questions for an association
Ask 10 to 14 once a year, a couple of months before renewals. Never skip value for the fee, which benefits members use, and renewal intent. Value and renewal predict membership income, and benefit use shows what members are actually paying for.
Member satisfaction survey questions ask how long people have been members, which benefits they use and value, whether membership is worth the fee and whether they will renew. These twenty two questions are built for a business or professional association, and split benefits used from benefits valued so the association knows what to keep.
The questions

Which questions to make required
Three are required: how long they have been a member, value for the fee and renewal intent. Tenure lets results be split between new and long standing members, who often want different things. Value and renewal are the measures that matter most for the association's income. Everything else is optional so busy members can finish quickly.

Three ways to change this bank
For a sports or social club, replace advocacy with facilities, coaching and social events, and ask about volunteering. For a professional body with continuing education, add questions on training quality and how easy it is to log points. For a not for profit membership, add whether members feel informed about how fees are spent and whether they would donate beyond the fee. In every version, survey members who did not renew as well, with a short separate form, because they explain the renewal number better than those who stayed. Keep the value and renewal questions identical each year, so the board can see whether changes to benefits or fees moved them.
Questions people ask
When should a member satisfaction survey be sent?
Once a year, a couple of months before most renewals, so there is time to act on what members say. Avoid the busiest season for members, such as end of financial year for business owners. Send a reminder after a week.
Why separate benefits used from benefits valued?
A benefit can be used out of habit but not valued, or valued but hardly used, like insurance. Knowing both shows what to promote, what to improve and what members would not miss. A matrix with used and valued columns captures both in one question.
How do you raise member survey response rates?
Send from a person members know, such as the president, keep it under ten minutes, say how long it takes and report back on last year's results. A prize draw can help if allowed. Members who see action on feedback respond more next time.
Should member surveys be anonymous?
Usually yes, with optional questions on business size or career stage for analysis. Anonymity makes members more honest about value and renewal. If you want to follow up with unhappy members, ask permission in a separate question and only contact those who agree.
What should an association do with low value scores?
Look at which members give them, by tenure and segment, and at which benefits they use. Often low value comes from members who do not know or use key benefits, which is a communication problem. Sometimes it means the fee or the benefits need to change.
Is renewal intent a reliable prediction?
It is a useful signal, especially the not sure group, who can still be won over. Stated intent tends to be more positive than actual renewals, so compare it with real renewal rates each year to calibrate. Follow up the not sure members personally.
How many members need to respond?
Enough from each segment you care about, such as new members and long standing ones, to see differences. For a small association, every response matters, so follow up personally. Report results by segment only where numbers are large enough to protect anonymity.

Sports club survey questions on fees, facilities and coaching
Sports club survey questions ask players, parents and volunteers whether fees are fair, how the facilities and coaching rate, how well the club communicates and whether people will stay. These twenty one questions start by asking each person's role, because a junior parent and a senior player experience a club very differently.
Volunteer application questions from interest to checks
A volunteer application has to match a willing person to a role and a roster, and find out early whether that role will need a background check. These questions put interests and availability first, where enthusiasm is highest, and leave checks and referees for the end, where a short explanation keeps people from walking away.
Resident satisfaction survey questions for an Australian council
Resident satisfaction survey questions ask people to rate the council services they use, say what should come first, and describe their last contact with the council. These twenty questions are built for an Australian local council, with a ten row service grid at the centre and a ward question so results reach the right councillors and wards can be compared fairly.
