Marketing request form that starts with the objective
A marketing request form tells the marketing team what kind of work is needed, what it should achieve, who it is for, the one message that matters, when, with what budget and who approves it. This agency version asks what should change because of the work before anything about format.

What each field on this form collects
A checklist of six channels records what is needed. Three required text fields follow: what should change because of the work, who it is for, and the one message they must take away. The deadline and budget share a row. An upload takes existing assets, and a dropdown names the approver.
Objective before output
Most internal marketing requests arrive as an output, such as a request for a flyer. The useful question is what should happen because of it. The second field asks exactly that, with a placeholder that is a number and a group of people. The American Marketing Association defines marketing in terms of creating and delivering value to customers, and a request that names the change it wants lets the team choose the channel that delivers it, which may not be the flyer at all.
One message, one approver
Campaigns fail when they try to say everything. The key message field asks for the one thing the audience must take away, in the requester's words. The approver dropdown solves the other common failure, a campaign that changes direction because a new person sees it late. If the request involves emails to customers, Australian businesses must also follow the Spam Act rules on consent and unsubscribing, which ACMA explains; the marketing team checks that before any email is sent.
How to adapt this form
A university adds the faculty and the student group. A franchise adds the store and whether the request needs head office brand approval. A charity adds the appeal and whether donor data will be used. A business with several brands adds a brand dropdown, so the team uses the right colours, voice and logo, and a question on whether legal review is needed for offers and competitions.
What makes this form work
What should change
The objective is a required box with a measurable example. The marketing team plans for an outcome, and can later report whether it happened.
One message
The one message field limits the brief to the single idea that matters. Every channel then carries the same message.
Approver named up front
A required approver from three roles. The person who signs off is known from the start, so the work does not change course late.
Questions people ask
What should a marketing request form ask?
What is needed, what it should achieve, who it is for, the key message, the deadline, the budget, any existing assets and who approves it. Asking for the objective first helps the marketing team suggest the right channel rather than just producing what was asked for.
How far ahead should marketing requests be made?
It depends on the work. A social post may take days, a multi channel campaign several weeks, and printed material needs time for printing and delivery. State lead times on the form or in the intro so requesters plan ahead.
Does an email campaign need consent?
In Australia, commercial emails need the recipient's consent, must identify the sender and must include a working way to unsubscribe, under the Spam Act. ACMA explains the rules. Check consent before sending any campaign that comes from a request like this.
Can requesters see the status of their request?
The form does not track status. The thank you screen says the marketing lead reviews requests twice a week. A webhook can post each request into a project tool where status is tracked, so the requester can follow it there instead of asking.
Why ask for a budget on an internal request?
Paid ads, printing and photography cost money, and knowing the budget early changes the plan. The field is optional, because many requests use only the team's time. A blank budget tells the team to plan for no spend, which rules out paid ads and printing from the start.
Who should approve marketing work?
One named person who owns the objective, usually the requester's manager or the head of the area. Fewer approvers means faster work. This form asks for the approver in the request, so the marketing team knows whose sign off counts.
What makes a good marketing objective?
A change you can count, in a group of people, by a date, such as 40 more workshop bookings from past clients by March. It tells the team what success looks like and lets them pick the cheapest channel that can get there. Avoid objectives that only describe activity, such as raise awareness, unless awareness can be measured.

Make one like thisOther form examples
Content request form for an editorial desk
A content request form gives an editorial team what it needs to decide on a piece and brief a writer: the format, the topic, the search phrase it should answer, who it is for, the sources, who asked and when it is needed. This newsletter publisher version schedules requests with clear sources first.
Design request form for an in house studio
A design request form gives a designer the deliverable, the size, the final copy, the brand to use, reference files, how many rounds of changes are expected, the deadline and the priority. This in house studio version requires final copy before work starts, because designing around placeholder text wastes a round.
Staff requisition form for a new role or a backfill
A staff requisition form asks approval to hire before a job ad goes out: which role, why it is needed, what it will cost and who signs off. This one separates a new position from a backfill, asks who is leaving when it is a backfill, and requires the position description as a PDF.
