Document request form for a school office
A document request form tells an office which record someone needs, who it is about, why and how to send it, with proof that the person asking is allowed to have it. This one does that for an Australian secondary school, with five documents to choose from, an identity upload and a standard or urgent turnaround.

What each field on this form collects
Name and an email or mobile share the first row. The document dropdown lists five records, from a semester report to a statement of results for a past year. Student name and year level is one short text, with a placeholder that also fits former students. Purpose is a two column radio of five. Delivery is a radio of email, collect or post. A paragraph explains who may request and what happens to the ID, then proof of identity is a required single file upload. Urgency is a radio of standard or urgent. All eight fields are required.
Why every request needs identity
School records are personal information about a student, and a request by email from someone claiming to be a parent is easy to fake. The ID upload lets the office compare the name with the school record before releasing anything. The paragraph above it says the office does not keep a copy once the request is done, which answers the question most people have before uploading a licence. Former students over 18 request their own records; a parent can request only while the student is under 18.
The Australian rules behind it
Under the Australian Privacy Principles, people can ask for access to personal information an organisation holds about them, and the organisation must check who is asking. Independent schools are covered by the Privacy Act, and Victorian government schools follow the state's school records policy, which sets how long records are kept and who can see them. Centrelink appears in the purpose list because enrolment letters are often needed for Youth Allowance claims.
How to adapt this form
A primary school drops the certificate and results options and adds a transfer letter. A TAFE or training provider replaces the list with a statement of attainment and a USI field. The office can turn on the email alert add-on so every request reaches the office inbox the moment it is sent.
What makes this form work
One field for current and former students
The student and year placeholder shows both Year 11 and left 2022. Former students fill the same form without a separate path.
The ID promise is next to the upload
The paragraph says the ID is checked and not kept. People upload a licence more readily when they know what happens to it.
Two speeds, both with days
Standard is 10 school days and urgent is 3. Requesters choose knowing exactly what each one means.
Questions people ask
Who can request school documents?
The student, and a parent or guardian while the student is under 18. The intro says so, and the identity upload is how the office checks. Former students can request their own records at any age. Requests from anyone else, such as an employer, need the student's written consent first.
How long does a school document request take?
The urgency radio sets the expectation: standard within 10 school days, urgent within 3. Most requests are copies of documents already on file and are quick; a replacement certificate can take longer. The thank-you screen repeats the standard time so nobody chases the office on day two.
Why ask what the document is for?
It helps the office send the right version. A visa application may need a certified copy or a letter on letterhead, while a job application usually only needs a PDF. The purpose also explains an urgent request without a second question. It does not change whether the document is released.
Is it safe to upload a driver licence?
The upload goes to the school's account with the request and is seen only by office staff. The paragraph above it says the office checks the ID against the school record and does not keep a copy afterwards. A requester can cover the licence number if the name, photo and date of birth are visible.
Does the requester get a confirmation email?
No. The thank-you screen confirms the request was received, and the office contacts the requester at the email or mobile given if anything is missing. The office itself can receive an email alert for each request, which is the add-on most school offices turn on first.
Can a former student request a document from many years ago?
Yes, as long as the school still holds it. Government schools keep student records under a set retention schedule, and some records, such as results, are kept far longer than day to day files. The year of leaving in the student field tells the office where to look. If a record no longer exists, the office says so.

Freedom of information request form for a council
A freedom of information request form has to describe the documents clearly enough for an agency to find them, say who is asking and how they want access, and deal with the application fee. This one does that for a Victorian council, with a date range to narrow the search, a hardship waiver option and the four forms of access.
Repair request form with photos and warranty check
A repair request form gives a service team what it needs to book a repair: which product, its serial number, what is wrong and whether the warranty covers it. This one adds optional photos of the fault, a proof of purchase upload for warranty claims, and a choice between dropping the appliance off and a courier pickup.
Software request form with a data and security check
A software request form lets IT check a new tool before anyone signs up: what it costs, how many people need it, what data it will hold and whether it works with company sign in. This one adds a required question about alternatives, so the request shows the team looked at what the company already pays for.
