Internship application form

An internship application has to do two things a job application does not: work for somebody with almost no employment history, and sort a large field quickly on things that are actually disqualifying. This one asks about study, availability and field readiness before it asks anybody to write 300 words. It is a classic form, because two PDF uploads and a referee table need one page where everything can be checked before sending.

There is no cut off mark. The panel reads the whole application, and a middling average with a season of field work behind it has beaten a high one without, more than once. Put the number in anyway, because leaving it blank is read as worse than whatever it is.

You can put yourself forward for a second stream and it does not weaken the first. Roughly a third of offers each year go to somebody's second choice, usually because a stream was oversubscribed rather than because the applicant was.

The next question decides how the placement is paid, so it is worth getting right. A placement that your course requires you to complete is treated differently from one you have chosen to do, and the difference is settled by your course rules rather than by anything we prefer.

What makes this form work

The disqualifying questions come before the writing

Right to work, availability and field readiness sit above the 300 word question. Somebody who cannot be there for any block finds out before they invest an evening, and the panel does not read essays from applicants it cannot hire.

The referee table is two fixed rows, not four loose fields

A table with name, role, email and relationship as columns collects both referees in one block, and the row count is set at design time because the field renders exactly the rows it was given. Two referees means two rows, which is also the instruction.

The mark question tells applicants what it is worth

A paragraph above it says there is no cut off, that the panel reads the whole application, and that a blank is read as worse than any number. That is the difference between a truthful average and a field left empty by the applicants who most need to be considered whole.

Questions people ask

What should an internship application ask that a job application does not?

Availability by block, expected completion, and whether the placement is a required part of the course. The last one is not curiosity: a placement a degree requires sits under different rules from one a student has chosen to do, and it changes how the position is paid. Asking it in one line beats discovering it at contract stage.

How many uploads is too many on an application form?

Two is the working limit. A CV and a transcript cover almost every panel's needs, and each upload field here takes one PDF up to 10 MB. Asking for a cover letter as a third file duplicates the written question the form already has, and every extra file is another reason for somebody to abandon the form on a phone.

Why put the swimming and licence questions in a grid?

Because they are the same question five times and each has the same two answers. A rating grid asks them in one block instead of five separate choice fields, and it reads as a checklist rather than an interrogation. In the responses table it lands in a single column as statement and answer pairs.

Can the form stop somebody applying after the closing date?

Not on a timer. There is no scheduled close on a form, so somebody has to unpublish it or switch its access away from public at 5pm. Saying the deadline on the form is honest, but the closing itself is a manual act and the page says so rather than implying a clock.

Gym membership form

A membership application for Ironbark Fitness that gets a new member from first visit to signed up in one sitting. Five sections cover personal details, an emergency contact, the membership plan and payment method, the seven question PAR-Q health screen as a yes or no grid, and a waiver signed on screen. About five minutes.

Open day registration form

An open day registration is not a headcount. It is the only chance an institution gets to find out what a visitor came for before they arrive, which is what turns a corridor full of strangers into a printed plan with room numbers on it. This one asks who the visitor is, what they want to look at and how many are in the car, in ten screens. It is conversational, because it is answered on a phone at a kitchen table with a course guide open.

Background check consent form

Consent that is buried on page four of an application is not really consent. This form stands on its own, lists each check as a separate line the candidate ticks or does not, collects the five years of addresses a police check actually needs, and sets out in plain words what the candidate can ask for afterwards. It is a classic form, because it carries an identity document upload and an inline signature, neither of which works in a conversational one.
