What makes this form work
One question per screen suits a phone
An RSVP link arrives by text or from a wedding website, so it is opened on a small screen with one thumb. Each of the eight questions gets its own page with a Continue button and a step counter, and only the first two are required. The GOV.UK design system recommends the same one thing per page pattern for this reason.
Declining is a first-class answer
The attendance question offers two choices, "Yes, count me in" and "Sadly I cannot make it", with the decline worded so nobody feels they owe an explanation. The party question's help text then says to skip it if you cannot come. A form that only really handles yes tends to get silence instead of a no.
Dietary needs are asked once, plainly
Meal choice and "Any allergies or dietary needs?" are two separate questions. The first is a tap between slow roasted lamb, pan fried barramundi and a wild mushroom risotto. The second is an open box, because the kitchen needs the actual allergy in the guest's own words rather than a tick.

What should an RSVP form ask?
Whether each person is attending, who is in their party by name, a meal choice if you are seating people, dietary needs, and a contact number for the day. A song request costs nothing and people enjoy it.
How do I stop people who decline seeing meal questions?
Open the Logic tab and add a rule to the attendance question: if the answer is Sadly I cannot make it, go to the last question or end and submit. A guest who declines then never sees the meal questions. Keep the skipped questions optional, because a required question the guest never reached still blocks the submission.
Is a conversational form better than a normal one for RSVPs?
For an RSVP, usually yes. It is short, it arrives on a phone, and the request is often unexpected, which is exactly when one question at a time helps.
What are the song request and message questions for?
The seventh question asks for one song that will get you on the dance floor, and the eighth is an open "Anything you would like to say to us?" box. Both are optional. They give the couple a playlist and a stack of notes, and they end the form on a warmer note than a phone number field would.
Should an RSVP form collect the names of everybody in the party?
Yes, if you are seating people. This form asks who is in your party straight after the yes, so the couple get names rather than a headcount. A number tells you how many chairs to hire. Names tell you how to build a table plan, and they catch the guest who quietly brought somebody.
When should a wedding RSVP form close?
Far enough ahead that the caterer deadline is not your problem. This form asks about 12 September, and the practical rule is to close three to four weeks before, then chase the silent guests by phone.
