Industry kit
Yoga studio templates for classes, retreats and the studio app
A yoga or pilates studio fills classes, screens new students, runs workshops and retreats, and keeps a timetable people can check on a phone. This kit links finished examples of each piece, written for invented Australian studios.
What a studio needs before the first class
A new student walks in with an injury history, a level of experience and a reason for coming, and the teacher needs to know all three before the class starts. A short intake form collects them, along with any condition that changes what poses are safe, such as pregnancy or a recent operation. A waiver records that the student understands the physical nature of the practice. Booking is the other half: a class booking form or app that shows the timetable and lets students reserve a mat before the room fills.
Workshops, retreats and the timetable
Most studios earn extra income from workshops, teacher training and retreats, which need their own registration with dates, rooms, dietary needs and deposits. The timetable changes every term and is what students check most, which is why many studios now offer it as an app. A flyer for the local cafe noticeboard and a pilates flyer for studios that run reformer classes complete the kit. The examples show each piece complete, so the questions, wording and layout can be read in full.
Who the kit is for
The kit suits independent yoga, pilates and mind body studios in Australia, from a single room to a studio with a reformer room. Yoga Australia is the peak body for teachers and publishes standards for teacher registration, and the national physical activity guidelines are a useful reference for class descriptions. The examples use invented studios and prices in dollars, and make no health claims for any style of practice. Each card opens the complete example, so a studio owner can judge the wording and the questions before using any of it.

A short intake beats a long one
Students fill in the intake on a phone in the doorway, so keep it to what the teacher needs: experience, injuries, health conditions, pregnancy and goals. Use yes or no questions with a detail field, and show the teacher the answers before class. A conversational form that asks one question at a time works well on a phone. The pilates intake in this kit shows a version that a new student finishes in under two minutes.
Retreats need more than a booking
A retreat registration is closer to a travel booking: dates, room type, sharing preferences, dietary needs, transport, emergency contacts and the deposit and cancellation terms. Put the cancellation rule next to the deposit, because that is where disputes start. The retreat registration example in this kit carries each of these, and the studio can follow up with an invoice for the balance from the proposals module.
The timetable on a phone
Students check the timetable more than any other page, so it should load fast and work on a phone. An app built in Sites shows the timetable and the class descriptions, and a test build installs it on a phone before it goes to the stores; publishing a website and running Expo builds, test or store, need a paid plan. Store submission is done from the studio own Apple and Google accounts, and the app example in this kit shows what the timetable screens look like.

Questions people ask
What should a yoga studio intake form ask?
It should ask for contact details, yoga experience, injuries, health conditions, pregnancy, recent surgery, goals and an emergency contact, plus acknowledgement of the waiver. Keep it short and let teachers see the answers before class, especially for students with injuries.
Do yoga studios need a waiver?
A waiver is common practice, because yoga is physical and injuries happen. It records that the student understands the risks and will tell the teacher about conditions. It does not remove the studio duty of care, so pair it with good screening and qualified teachers.
How do studios take retreat deposits?
Most take a deposit on registration and the balance several weeks before the retreat. State the deposit, the balance date and the cancellation rule on the registration form. A deposit invoice with a pay link can be issued from the proposals module; card payments need a paid plan and a connected Stripe account.
Can a yoga studio have its own app?
Yes. Sites can build a phone app with the timetable, class descriptions and studio information. Test and store builds both need a paid plan, and store submission is done from the studio own developer accounts. The yoga studio app in this kit is an example.
Should pilates and yoga use different forms?
The core intake is similar, but reformer pilates often asks more about injuries and goals because equipment changes the risk. Studios running both can use one intake with an extra pilates section, or keep two short forms as the examples in this kit do.
